RN Nurse Navigator, Orthopedic Services at Golden Valley Memorial Hospital – Clinton, Missouri

The orthopedic nurse navigator is responsible for the overall coordination and implementation of Golden Valley Memorial Healthcare's orthopedic program. The navigator will guide the orthopedic patient and their family from preoperative education through the continuum of care. This individual will interact with patients and their families, physicians and their care partners as well as several key hospital departments including surgery, dietitian, rehabilitation and occupational therapy, nursing, case management, marketing, home health and others involved in the care of the orthopedic patient. The nurse navigator will regularly collect data through chart review and computer-generated reports and present data and reports to appropriate committees and other leadership groups as directed. This individual will develop, facilitate, and monitor selected performance metrics and ensure compliance with all activities required to achieve and maintain disease-specific certifications. The orthopedic nurse navigator will serve in a highly supportive and personal manner to ensure all orthopedic patients receive the resources and services they need. Effective communication, organization, compassion, and the ability to provide individual attention to each patient are critical to the success of this position.

Requirements:
MO RN license in good standing
One to three years' experience
