Occupational Therapist Home Health at Vanura Homehealth Services Inc – Rancho Cucamonga, California

Coverage areas - Temecula, Murietta, Colton, Menifee

- Provide initial and ongoing assessment to determine level of functioning.
- Responsible in gathering background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient's physical and psychosocial functioning.
- Responsible in developing and revising the plan of care in consultation with the physician and other care-team members.
- Responsible for planning, evaluation and treatment implementation of assigned caseload.
- Responsible in preparing clinical progress notes.
- Responsible in supervising therapy assistants and home health aides as appropriate.
- Responsible for evaluating outcomes.
- Adhere to all Company policies and procedures.

Knowledge, skills and experience
- Minimum education (or substitute experience) required: Currently licensed by the California Board of Occupational Therapy.
- Minimum experience required: Minimum of one year in acute care setting. Two years experience in Home Health preferred.
