Outpatient Pharmacy Resident, PGY1 at Holzer Health System – Gallipolis, Ohio

Works under the supervision of the Business Operations Manager/Director/Clinical Pharmacist of Holzer Family Pharmacy. Evaluates medication therapy, makes medication therapy recommendations, performs medication reconciliation, follows high risk patients in ambulatory setting, collaborates with discharge process in medication provision and education. Fills, compounds, and dispenses pharmaceuticals as prescribed by the physician in accord with all legal requirements and to provide pharmaceutical information to patients and allied health professionals. Greets and waits on the general public personally or via telephone to serve their needs for drugs and medical supplies. Develops necessary documentation for ASHP accreditation of residency program.

Performs other duties as assigned.
