Job Description
Are you a Licensed Practical Nurse (LPN), Licensed Vocational Nurse (LVN), or Registered Nurse (RN) passionate about proactive, patient-centered care?
Nsight Health, Inc. is seeking a Remote RPM Nurse to join our dynamic Contact Center team. In this role, you'll monitor and manage patient alerts, conduct proactive outreach to improve outcomes, and provide education to support ongoing care. You'll also handle both inbound and outbound calls, routing them based on clinical urgency and patient needs.

Shift Differential Pay –
$1.50/hr for all hours worked after 7:00 PM EST
$1.50/hr for all hours worked on Saturdays and Sundays

Key ResponsibilitiesProactively manage high volumes of outbound patient calls
Monitor and respond to RPM alerts with appropriate follow-up
Conduct monthly wellness assessments and chart reviews
Coordinate care and escalate issues to healthcare providers
Accurately document interactions in our clinical software
Meet performance metrics and patient engagement goals
Support a compassionate, team-focused remote work environment
Active LPN, LVN, or RN license (Required)
Strong empathy, communication, and patient education skills
Moderate to advanced computer literacy
Reliable, high-speed internet and HIPAA-compliant workspace (speed test required during interview)
