Job Description
Under general supervision is responsible for providing speech, language, and dysphagia services to patients include; evaluations, planning individual patient care programs, treatments, and home
programs.
Essential Duties and Responsibilities:

Education: Master’s degree in speech pathology.
Licensure/Certification: Current licensure by the Indiana Board of Examiners on Speech Pathology and Audiology required.
Experience: Two - three years’ experience in clinical speech pathology. Excellent interpersonal skills required. May be assigned to work in homes of patients from all socio-economic levels.

Essential Duties and Responsibilities:
- Performs patient evaluations and administers treatment for speech language and swallowing disorders.
- Designs and revises plans of care and goals.
- Provides written and/or verbal reports to physicians.
- Completes documentation according to unit standards and regulatory requirements.
- Participates in in-service education programs.
- Provides instruction, direction and supervision to new employees.
- Keeps abreast of current trends and developments in speech and swallowing disorders
- Participates in related activities such as patient care committees, case conferences, personnel
orientations, community education projects and in-service education. - Attends staff meetings.
- Provides families of patients with information concerning specific disorders as needed
- Review each patients chart and history to become familiar with patients’ needs
- Develops patient literature with other members of the department
- Maintains contact with all referring physicians of patients in the programs and sends discharge summary to the physician after program is completed
- Assists patients to obtain home therapy equipment
- Attends and contributes to in-services and comprehensive plans of care for the patients of the Agency
- Assists with quarterly clinical records review
- Uses equipment and supplies efficiently and effectively
- Provide reliable transportation to and from work and patients’ homes
- Pursues personal professional growth and development through participation in education programs and review of current health care literature
- Understand and adheres to all policies, philosophies and standards of the Home Health Care Agency
- Any other duties as assigned
