Job Description
Summary
This position is established for the purpose of performing assessment and screening to link individuals to needed services.
Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Conducts phone calls and in-person contacts with individuals in accordance with program specifications and time frames.
- Assists individuals in gaining access to any necessary services regardless of funding source.
- Establishes initial service needs and ensures the prevention of service duplication.
- Networks with informal caregivers to obtain necessary information and coordinate formal and informal services as appropriate.
- Conducts Level of Care and Comprehensive assessments as needed.
- Maintains all appropriate documentation in formats and time frames prescribed by the PAA, OHCW, ODA and the Ohio Department of Medicaid.
- Maintains job functions as provided by and unique to this distinct work unit, separate from other units and reporting up a separate supervisory chain.
- Understands the Agency’s Conflict of Interest Policy and complies with the Ohio Aging Network’s requirements for disclosure.
- Participates in training to maintain and improve job skills.
- Assists with and completes special projects and activities as assigned.
- Performs duties with respectful communications & behaviors.
- Ability to work from home.
- Ability to relate well with all individuals, community agencies, and Area Agency staff.
- Strong verbal and written communication skills.
- Knowledgeable in Microsoft Word and Excel.
- Ability to communicate professionally, respectfully and effectively.
- Ability to make sound decisions and demonstrate critical thinking skills.
- Ability to prioritize, organize and maintain accuracy in a flexible job environment.
- Successful completion of the Ohio Department of Aging certification required within 6 months of employment.

While performing duties of this job, the employee is frequently required to:
Carry job related items weighing up to 30lbs into private homes and facilities which may not be readily accessible.
Stand; walk; sit, type, talk
HIPAA/Confidentiality
Must maintain strict confidentiality and comply with all Agency HIPAA policies and procedures.
Travel
Ability to travel daily in a multi-county service area and access private homes and facilities which may not be readily accessible.
Eligibility Requirements
Registered Nurse or Licensed Social Worker holding an active license to practice in the State of Ohio.
