Required Qualifications:
SPECIAL CERTIFICATIONS AND LICENSES:
Requires a valid driver’s license and a driving record that is in compliance with City Driving Standards.
Depending on assignment, may require Qualified Mental Health Professional (QMHP) or Qualified Intellectual Disability Professional (QIDP) certification and highest Part C standards within 6 months of hire; CPR, First Aid and medication administration certifications within 3 months of hire, Certified Substance Abuse Counselor (CSAC) certification upon hire, Case Management Modules and Intellectual Disabilities Workbook completed within thirty days of hire, Part C Early Intervention Certification within six months of hire, and/or Pre-Screening Certification within 6 weeks of hire (12 weeks for part-time staff).

The Clinician II in our Intellectual/Developmental Disability Case Management Services Division serves persons diagnosed with an intellectual or developmental disability.
They will facilitate the coordination, communication, and collaboration between individuals, families, service providers, and agencies for the purpose of supporting the individual.
The Clinician II duties include but not limited to:
The Clinician II in our Intellectual/Developmental Disability Case Management Services Division serves persons diagnosed with an intellectual or developmental disability.
They will facilitate the coordination, communication, and collaboration between individuals, families, service providers, and agencies for the purpose of supporting the individual.
The Clinician II duties include but not limited to:
- Develop individual service plans and implement plans in compliance with State Licensure and Medicaid regulations and standards.
- Conduct thorough assessments, prepare well-written service plans, reports, and progress notes, and maintain individual records in the electronic medical records system.
- The Clinician II will also assist individuals by linking, locating, and monitoring services, resources, and benefits, and making collateral contact community partners to promote the implementation of the individual service plan.
- Identify and utilize community resources, complete benefit applications, and provide assistance if necessary to get the individual to the resource or service provider.
- Coordinate services with other agencies and providers involved with the individual and maintain necessary records to document the provision of case management services.
- Clinician II’s monitor service delivery and make home and other agency site visits to observe and evaluate individuals’ responses to their environment.

VOCATIONAL/EDUCATIONAL REQUIREMENT:
Requires a bachelor’s degree in social work, counseling, psychology, or a closely related field or other degree as approved by the Virginia Department of Health Professions. A master’s degree in Social Work, Counseling, or Rehabilitation is required for some assigned programs.
EXPERIENCE REQUIREMENT:
In addition to satisfying the vocational/educational standard, this class requires a minimum of two years of related, full-time equivalent experience.
