Social Work Case Manager - Flexible Hybrid Position
Utah Case Management is looking for compassionate, motivated Social Work Case Managers to join our growing team and support elderly and mental health clients through Utah Medicaid’s New Choices Waiver (NCW) program.
This is a flexible hybrid position that combines meaningful, in-person work with the convenience of remote documentation. You’ll have the opportunity to build relationships with clients, advocate for their needs, coordinate important services, and help them successfully navigate the resources available to them. We are estimating this position to be around 20-40 hours per week.
What You’ll Do
As a Social Work Case Manager, you will help coordinate services and support for elderly and mental health clients participating in the New Choices Waiver program.
Your responsibilities will include:
- Maintain monthly face-to-face contact with assigned clients.
- Visit clients in their homes, assisted living communities, or long-term care facilities.
- Assess client needs and help coordinate appropriate services and resources.
- Communicate regularly with clients, families, service providers, facilities, and other members of the care team.
- Advocate for clients and help ensure their services are meeting their needs.
- Complete, update, submit, and maintain required client documentation.
- Communicate with team members to coordinate and support client services.
- Attend regular staff meetings and required company trainings.
- Manage your caseload independently while meeting documentation, scheduling, and quality expectations.
- Use your own computer/device for remote work and documentation.
- Travel locally to complete required monthly client visits.
What We’re Looking For
We’re looking for someone who is compassionate, dependable, organized, and comfortable working independently.
Required qualifications:
- Utah resident currently residing in the SALT LAKE COUNTY.
- Current Utah SSW, CSW, LCSW, or RN license.
- Ability to travel locally for monthly client visits.
- Availability to complete required in-person client visits during normal business hours.
- Comfortable working independently while also being a collaborative member of a team.
- Strong communication and organizational skills.
- Detail-oriented and able to complete documentation accurately and on time.
- Self-motivated and able to manage your own schedule and caseload.
- Strong ability to build and maintain positive relationships with clients, families, facilities, and service providers.
- Reliable access to your own computer/device for the position.
Compensation
Billable client work: Up to $26/hour
Billable time includes eligible client-related work that can be billed to Medicaid.
Non-billable work: $15/hour
Non-billable time includes position-related activities that cannot be billed to Medicaid, such as staff meetings, training, and other administrative activities.
Because this position offers significant scheduling flexibility, your total compensation will depend on your caseload, billable hours, and the amount of non-billable work required.
