About the Role
Avail Health is seeking an experienced Mobile Registered Nurse (RN) to provide in-person clinical assessment services for Medicare Advantage Institutional Special Needs Plan (I-SNP) members residing in long-term care facilities within the assigned service area.
The role includes two primary areas of clinical service: independently completing Health Risk Assessments (HRAs) and related documentation to support member-driven Individualized Care Plans (ICPs), and partnering with Avail Health Nurse Practitioners to support Chronic Disease Assessments (CDAs) through in-person clinical assessment and facilitation of virtual NP visits.
This opportunity is well suited for an experienced RN who is comfortable providing services independently in long-term care environments, engaging members and their authorized representatives, and collaborating with NPs, facility staff, and interdisciplinary care teams.
The contractor will exercise independent professional and clinical judgment in performing accepted clinical services within RN scope of practice and applicable clinical, payer, regulatory, facility, privacy, security, and documentation requirements.
Clinical Services
Health Risk Assessments & Individualized Care Planning
Independently conduct in-person Health Risk Assessments (HRAs) with I-SNP Medicare Advantage members residing in long-term care facilities.
Complete comprehensive assessments addressing applicable medical, functional, cognitive, behavioral health, psychosocial, and other required assessment domains.
Document assessment findings accurately and thoroughly to support development and ongoing maintenance of member-driven Individualized Care Plans (ICPs).
Engage members in discussions regarding their health needs, goals, preferences, and priorities to help ensure individualized care planning reflects what matters to the member.
Communicate with and engage authorized representatives (ARs) when applicable, including when AR participation is needed to complete assessment or care-planning activities.
Coordinate with facility staff and members of the interdisciplinary care team as clinically appropriate to obtain relevant information and support assessment and care-planning activities.
Identify potential changes in condition, care gaps, or unmet needs and communicate findings through established clinical escalation and care coordination processes.
Complete required HRA and supporting clinical documentation accurately and within applicable timeframes.
Chronic Disease Assessments & NP Collaboration
Partner with Avail Health Nurse Practitioners to support Chronic Disease Assessments (CDAs) for I-SNP members.
Obtain required clinical information, vital signs, and designated screening information in support of the assessment.
Facilitate virtual NP encounters, including technology setup and real-time coordination among the member, facility team, and NP.
Provide on-site clinical support during virtual encounters, including physical assessment components within RN scope of practice and applicable clinical protocols.
Communicate relevant clinical findings and follow-up needs to the NP and other members of the care team as clinically appropriate.
Complete applicable clinical documentation and support timely completion of accepted CDA services.
Mobile Clinical Services
Provide services at long-term care facilities within the designated service area for accepted assignments.
Coordinate accepted member and facility assignments efficiently and complete associated services and documentation.
Establish effective working relationships with members, authorized representatives, facility staff, NPs, and interdisciplinary care team members.
Maintain member privacy and comply with applicable HIPAA, payer, facility, credentialing, clinical-quality, patient-safety, privacy, and security requirements.
Use approved technology, applications, and equipment as required for secure access to clinical information and completion of services.
Qualifications
Required
Current, active, unrestricted, and unencumbered West Virginia Registered Nurse license or qualifying multistate compact RN license, as applicable to the service location.
Minimum of 3 years of clinical nursing experience working with older adults and/or individuals with chronic or complex medical conditions.
Strong clinical assessment and documentation skills.
Ability to work independently and exercise appropriate professional and clinical judgment within RN scope of practice.
Strong interpersonal and communication skills, including the ability to engage effectively with older adults, authorized representatives, facility staff, and healthcare professionals.
Strong organization and time-management skills with the ability to coordinate accepted services across multiple members and facilities.
Comfort using electronic clinical documentation systems, mobile devices, and telehealth technology.
Reliable transportation and ability to travel to long-term care facilities within the designated service area.
Preferred
Long-term care, skilled nursing, geriatric, or post-acute care experience.
Experience conducting Health Risk Assessments (HRAs) or other comprehensive nursing assessments.
Medicare Advantage and/or I-SNP experience.
Experience with individualized care planning or interdisciplinary care management.
Risk adjustment or chronic disease assessment experience.
Prior mobile, field-based, or multi-facility nursing experience.
Contract & Availability
This is a 1099 independent contractor, PRN engagement. The initial contract period is anticipated to be October 2026 through December 2026, with the potential for extension based on program needs and mutual agreement.
The contractor will independently determine and communicate their availability and service capacity. Avail Health may offer member assessments, facility assignments, or other services within the scope of the engagement based on program needs and contractor availability. The contractor may accept or decline offered assignments.
Once an assignment is accepted, the contractor is expected to fulfill the applicable clinical, documentation, continuity, and contractual responsibilities associated with the accepted services.
Services are primarily available during daytime hours based on member and facility availability. The parties may coordinate anticipated assessment volume and service needs based on member needs, facility access, and contractor availability.
No minimum number of assignments, hours, or compensation is guaranteed.
This is a mobile engagement requiring travel to long-term care facilities within the Morgantown, West Virginia service area.
Compensation
Clinical Services Rate | $60.00 per hour |
Non-Clinical Services Rate | $35.00 per hour |
Mileage Reimbursement | $0.725 per mile for eligible travel over 30 miles per day, up to $150 per day |
Overnight Travel | Approved hotel accommodations + $50 per-night travel allowance when an accepted assignment requires overnight travel |
Required privacy, security, compliance, patient-safety, technology, clinical-system orientation, or other required non-clinical activities necessary to qualify the contractor to provide services or obtain required system access will be compensated at the Non-Clinical Services Rate.
Additional Requirements
Engagement is contingent upon successful completion of applicable background screening, healthcare screening, exclusion screening, credentialing, privileging, and other requirements necessary to qualify the contractor to provide services.
Maintain all licenses and professional credentials required to perform accepted services in good standing throughout the engagement.
Reasonably cooperate with applicable credentialing, privileging, payer enrollment, recredentialing, and related requirements.
Do not begin clinical services until applicable credentialing, privileging, payer, facility, or other required approvals have been completed and Avail Health confirms eligibility to begin services.
Complete required privacy, security, compliance, patient-safety, technology, and clinical-system orientation.
Use Avail Health-managed devices, accounts, applications, or systems when required to protect PHI or confidential information or satisfy applicable privacy, cybersecurity, regulatory, payer, facility, or contractual requirements.
