Job Summary
The LPN is knowledgeable of patient’s home health care plan and delivers nursing interventions, treatments, and education to the patient under the supervision of the Registered Nurse. The LPN communicates patient status to other IDT members to ensure that appropriate medical care or care plan changes are appropriate and timely.
Core Duties:
- The interventions, treatments, and patient education are delivered in accordance with medical orders, standards of care, safety practices, patient informed consent and geared towards the patient’s comprehension level.
- The patient’s satisfaction with and response to treatment are identified when delivering the treatment plan and is communicated to the supervising registered nurse.
- Team communication and documentation occur within agency timeframes and guidelines, is available to all appropriate members of the patient service team and is reflective of the patient status, needs, goals, and plan.
- Provider orders are current and consistent with the plan of care and documented in the medical record.
- Communicates patient information and change in status to provider as appropriate.
- Nursing services are delivered to meet patient satisfaction goals.
- Participates in quality activities to improve service and operation to achieve organizational outcomes.
- Work is completed in accordance with productivity standards.
- Work is completed within reimbursement source guidelines for efficiency and fiscal responsibility.
- Adheres to scheduling guidelines to meet patient and agency needs.
Minimum Qualifications:
Graduate of an accredited professional licensed practical nurse (LPN) program
Required Certifications, Licensure:
- Current license in the State of Rhode Island as a Licensed Practical Nurse.
- Current AHA CPR certification
- Valid state issued driver’s license
- Proof of current liability car insurance
Preferred Qualifications/Additional Skills:
- Minimum two years direct patient care experience
