JOB TITLE: Social Worker/Bereavement Counselor/Volunteer Coordinator - MSW or LCSW
DEPARTMENT: Home Health/Hospice
REPORTS TO: Home Health & Hospice Director
JOB SUMMARY
HOSPICE
The Social Worker/Bereavement Counselor provides supportive and psychosocial services to help meet social, emotional and economic needs of terminally ill patients and family members enrolled in the Hospice Program at Cody Regional Health. The Social Worker is a member of the Hospice Program Interdisciplinary Team who provides direct and indirect services to the patients, caregivers, families and the bereaved in accordance with the Medicare Condition of Participation and departmental policies and procedures. The Hospice Social Worker/Bereavement Counselor serves as a core member of the Interdisciplinary team and collaborates with other members of the IDT to ensure quality and safe patient care for the patients and families receiving Hospice services.
HOME HEALTH
The Social Worker provides supportive and psychosocial services to resolve social or emotional problems that interfere with the effective treatment of the Home Health patient’s medical condition or rate of recovery. The Social Worker collaborates with other members of the Home Health Care team and assists the patient with community resources linkages as needed.
The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status. They must be able to interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures.
JOB REQUIREMENTS
Minimum MSW in Social Work degree with LCSW license, or ability to obtain license.
EXPERIENCE
Minimum of two years supervised clinical experience in an interdisciplinary health care or a related field preferred. Supervisory experience preferred. Experience with bereavement counseling preferred. Experience with volunteer programs preferred.

- Must have an understanding of patient’s needs.
- Must be able to exhibit compassion during assessment, and while dealing with family members.
- Must possess a high degree of confidentiality in all matters with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
- Must have strong interpersonal skills with the ability to communicate with staff and public in a friendly, courteous and helpful manner.
- Must have well-developed verbal and written communication skills.
- Effective computer and keyboarding skills.
- Must have ability to learn and use Microsoft Office Suite products including Word and Outlook.
- Ability to learn new computer software applications.
