JOB SUMMARY: The incumbent must be knowledgeable in Rehabilitation Admission Criteria per CMS Guidelines and also knowledgeable in the Pre-Admission Screening process. They shall also demonstrate accountability for and contribution to program development, quality improvement, and problem solving and productivity enhancement in a flexible interdisciplinary fashion. Reports to the Patient Care Manager-4A/4B but ultimately responsible to the Inpatient Rehab Director for meeting program standards. Main duties include: 1) Respond to and carry out preadmission screenings for patients referred to the rehab program, 2) to develop and implement a referral development program as a means to promote the features and benefits of the rehab program to patients, families and community members and 3) initiates the flow of patient information to the Rehab Managers, Program Director and Medical Director for review and decision-making. Manages all referrals, admissions and denials with a proactive approach. Ancillary duties include: maintain current knowledge of insurance and/or payer source regulations; participation in quality improvement activities; maintain effective relationships with physicians and all other staff members to coordinate patient care; attend in services and meetings as required. Admissions Coordinators will provide Clinical Updates to patient’s payer, as required by patient’s payer, for the patients whose admission to the Rehab Unit, each Admission Coordinator facilitates.

PERFORMANCE EXPECTATIONS: Includes the following: Other duties may be assigned.

• Demonstrates the ability to perform assessments of patients referred for inpatient rehabilitation services.
• Demonstrates the ability to make recommendations for alternate levels of care within the continuum to referral sources if the patient is not a candidate for admission.
• Demonstrates a working knowledge of how payment sources impact the admissions process and is proactive in identifying patient needs accordingly.
• Demonstrates the ability to initiate the admissions of patients by fully completing necessary forms and insurance pre-certifications.
• Demonstrates knowledge of Medicare acute rehabilitation unit admission regulations and applies them appropriately in the preadmission screening & assessment process.
• Demonstrates the ability to promote the acute inpatient rehabilitation program, creating trusting, consultative relationships at all levels within the organization as well as with external facilities.
• Demonstrates the ability to communicate effectively and quickly with all referral sources at every stage of the referral process as a means to achieve referral source satisfaction and to advocate for patients by providing the fastest decision regarding admission.

QUALIFICATIONS:

Education/Skills: The incumbent shall, at a minimum, be a licensed clinician and a graduate of a non-online program (RN, PT, OT, SLP).

Work Experience: Must have a minimum of 2 years of clinical experience in which clinical expertise and strong interpersonal skills have been demonstrated; prefer experience in a rehab setting. Experience with computer and other electronic data entry devices. Must have a valid driver’s license.

Certification/Licensure-DUE UPON HIRE

• Licensed RN able to practice within the State of MS

• Or Licensed PT, OT or SLP able to practice within the State of MS

Additional Certification/Licensure - Obtained based on required timeframe below
