Job Description
Do you believe healthcare is about more than treating illness?
At Bridge Community Health Clinic, we know that a patient's health is shaped by much more than a diagnosis. Transportation, housing, food security, finances, and access to resources all play a role in achieving better health. As a Care Coordinator, you'll help patients overcome these barriers while working alongside an incredible interdisciplinary team that is committed to whole-person care.
This is more than a care coordination position—it's an opportunity to build meaningful relationships, advocate for patients, strengthen community partnerships, and make a lasting impact every day.
What You'll Do
As our Care Coordinator, you'll be an essential member of our integrated healthcare team by:
• Coordinating care for patients with complex medical, behavioral health, dental, and social needs.
• Helping patients navigate healthcare services, insurance, financial assistance, and community resources.
• Connecting patients with housing, transportation, food assistance, medication programs, and other community supports.
• Identifying barriers to care and developing solutions that improve patient outcomes.
• Collaborating with Primary Care, Behavioral Health, Dental, and community partners to ensure coordinated, patient-centered care.
• Providing ongoing patient outreach and follow-up to encourage engagement and successful treatment.
• Advocating for patients while empowering them to become active participants in their own healthcare.
• Building and maintaining relationships with community organizations to expand resources available to our patients.
• Participating in quality improvement initiatives that enhance patient care and clinic operations.
• Supporting operational continuity by providing cross-coverage for the Referral
Coordinator and assisting the Behavioral Health Consultant with non-clinical patient navigation and resource coordination.
What We're Looking For
We're seeking someone who is:
• Passionate about helping others.
• A natural relationship builder and collaborator.
• Organized, adaptable, and able to manage multiple priorities.
• Skilled at problem-solving and thinking creatively.
• Compassionate while maintaining professional boundaries.
• Comfortable working independently and as part of a multidisciplinary healthcare team.
Qualifications
• Bachelor's degree in Human Services, Social Work, Psychology, Public Health, Healthcare Administration, or a related field required.
• Experience in healthcare, case management, care coordination, behavioral health, or community services preferred.
• Knowledge of community resources and social drivers of health preferred.

• The opportunity to make a lasting impact on individuals and the health of our community.
