Job Description

Location: Stephen Klein Wellness Center | 2144 Cecil B. Moore Avenue, Philadelphia, PA 19121

Schedule: Full-Time | Monday-Friday | 8:30 AM - 4:30 PM

Pay: Up to $26.52/hour, based on experience

Project HOME is seeking an experienced, compassionate Certified Peer Specialist to join our Medication for Opioid Use Disorder (MOUD) team. In this role, you will provide peer-based recovery support to individuals living with opioid use disorder and other complex behavioral health and social needs, helping them navigate recovery while overcoming barriers related to healthcare, housing, employment, education, and community resources.

Working within Project HOME's integrated healthcare model, you'll partner with physicians, behavioral health clinicians, nurses, social workers, and care managers to deliver coordinated, person-centered care that promotes long-term recovery and wellness.

What You'll Do

Recovery Support

  • Build trusting relationships with patients throughout their recovery journey.
  • Develop individualized recovery, relapse prevention, and harm reduction plans.
  • Support patients in setting and achieving meaningful recovery goals.
  • Provide peer support and recovery coaching from engagement through long-term recovery.
  • Promote self-advocacy, wellness, resilience, and independence.

Care Coordination

  • Collaborate with medical, behavioral health, and care management teams.
  • Connect patients with housing, employment, educational, and community resources.
  • Accompany patients to healthcare and social service appointments as needed.
  • Participate in interdisciplinary care coordination and MOUD team meetings.

Documentation & Quality

  • Complete timely documentation, progress notes, screenings, and required program records.
  • Support quality improvement initiatives through accurate documentation and reporting.
  • Participate in ongoing training, supervision, and professional development.

What You'll Need

  • High school diploma or GED.
  • Current Certified Peer Specialist (CPS) or Certified Recovery Specialist (CRS) certification.
  • 1+ years of experience supporting individuals in behavioral health, substance use treatment, recovery services, or direct care settings.
  • Lived experience as a person in recovery.
  • Experience using harm reduction, trauma-informed, and person-centered approaches.
  • Ability to build trusting relationships while respecting each person's unique recovery journey.

What Experience You May Bring

  • Experience working with individuals experiencing substance use disorders, homelessness, trauma, and mental health conditions.
  • Knowledge of Philadelphia behavioral health, recovery, housing, and community resources.
  • Experience in integrated healthcare, primary care, or behavioral health settings.
  • Experience facilitating peer recovery groups.
  • Crisis intervention and de-escalation skills.
  • Bilingual (English/Spanish).
