Job Description
The purpose of the RN Staff Nurse – Assertive Community Treatment (ACT) Team role is to provide comprehensive nursing care and clinical support to individuals with severe and persistent mental illness and co-occurring conditions within a community-based treatment setting. This role is responsible for managing and administering medications, conducting clinical and psychiatric assessments, addressing physical health needs, and coordinating care with medical providers and the interdisciplinary ACT team. The position supports the delivery of individualized treatment, rehabilitation, and recovery focused services to promote stability, health, and independent functioning in the community.
MAJOR RESPONSIBILITIES
DIRECT DUTIES:
- Administers and manages medications and treatments
- Conducts psychiatric, clinical, and physical health assessments
- Provides nursing care and monitors client health status
- Completes vital signs, metabolic screenings, injections, and ordered treatments
- Provides in-home services (ADL training, medication training, home management)
- Delivers supportive counseling to clients
- Coordinates emergency and crisis intervention services
- Ensures follow-through on medical appointments and prescribed care
- Educates clients on symptoms, medications, and disease prevention
- Provides direct support and consultation to families
OTHER RESPONSIBILITIES:
- Participates in treatment planning and interdisciplinary case conferences
- Refers and links clients to external services and providers
- Coordinates housing, employment, and community resource development
- Documents clinical interactions and communicates updates to team members
- Ensures compliance with licensing regulations and medication laws
- Manages infectious disease control and compliance activities
- Maintains relationships with external providers (pharmacy, medical, ancillary services)
- Trains and supervises unlicensed staff on care procedures and standards
- Maintains medication inventory and ensures proper storage and documentation
- Attends meetings, trainings, and team coordination activities
- Communicates treatment or condition changes to staff and providers
Responds to on-call responsibilities - Advocates for clients within the community
JOB QUALIFICATIONS AND COMPETENCIES
Requirements:
- Must have and maintain current licensure as a Registered Nurse in Delaware
- Pre-employment Tuberculin Skin Test and CPR Certification required
- Must maintain full security clearance approved by the Delaware Department of Correction
- Successful completion of a background investigation
Education and Experience:
- Graduate of an accredited School of Nursing
- Bachelor of Science in Nursing (preferred)
- Minimum one year of clinical experience (preferred)

The Assertive Community Treatment (ACT) Team is an interdisciplinary team that ensures a recovery-oriented system of care, with a full range of treatment functions, 24 hours a day, 7 days week. The program employs master’s and bachelor’s-level counselors, a registered nurse, a substance abuse specialist, a vocational specialist, a psychiatrist/APN, a peer support specialist, and program assistant. Team members work together with clients to promote symptom stability and appropriate use of medication; restore personal, community living, and social skills; promote and maintain physical health; establish access to entitlements, housing, work, and social opportunities; and promote and maintain the highest possible level of functioning in the community. This program is funded by and part of the system of public services offered by Delaware DSAMH.
