Description:

Licensed Clinician (LPC, LCPC, LMSW, LCSW, LMFT)

Assumes responsibility as clinician for co-occurring disorders and Substance Use Disorder per The Walker Center policies and procedures.

Walker Center is open 24 hours per day, 365 days per year. The Licensed Clinician job classification is expected to work assigned schedule ensuring daily coverage by clinical staff and be present at staff meetings, deliver morning and afternoon lectures, attend weekly interdisciplinary team meetings and have all documentation completed prior to the end of the day.

ESSENTIAL FUNCTIONS:

  • Completes client evaluation based on assessment of their needs and complete diagnosis based on DSM 5 criteria and level of care placement based on ASAM Criteria.
  • Completes individualized treatment planning with clients including initial, on-going, continuing care and discharge plan.
  • Integrate trauma informed, person centered therapeutic interventions to clients and families.
  • Completes referral and discharge planning with client and family, or oversees referral completion with the Discharge Planner, if available.
  • Completes timely insurance reviews (or communicates needs to supervisor or treatment team).
  • Completes individual, group, and family counseling sessions which address the individual mental health needs of the client, including relapse prevention.
  • Prepares and completes all necessary documentation and record keeping.
  • Prepares individualized daily notes, showing accurate and personalized documentation regarding the client’s progress and / or lack of progress.
  • Prepares and conducts lectures on substance use disorders and co-occurring disorders.
  • Maintains knowledge of and adheres to The Walker Center’s policies and procedures.
  • Demonstrates knowledge and competency related to confidentiality and ethical standards of the counseling profession.
  • Attends continuing education classes and applies knowledge learned to work at The Walker Center. Shares knowledge with co-workers.
  • Trains, mentors and guides entry level / training counselors as assigned.
  • Works within a team setting- works effectively with a treatment team format, knows how to work with supervision.
  • Collect data and comply with quarterly review of surveys for distribution to Outpatient Business Office Manager, Outpatient Treatment Coordinator and Residential Treatment Coordinator.
  • Be familiar with all policies and procedures and notify supervisors of any policy non-compliance.
  • Contact discharged clients to gather survey results at 30 days, 6 months and 1-year intervals and complete documentation of successful or unsuccessful contacts for both residential and outpatient.
  • Read annual CARF and ASAM updates, check for compliance, provide suggestions and participate in trainings as needed.
  • Logs and tracks accurate data of survey results.
  • monitor and report on performance, measure against set indicators and coordinate with other managers and staff throughout the organization to ensure that the quality assurance system is functioning properly.
  • Conducts Electronic Health Record audits with written reports to supervisors.
  • Formulates and prepares written and/or oral reports, summaries and/or analysis of findings and presents to supervisor.
  • Where appropriate, advise on changes and identifies training, tools and techniques to enable others to achieve quality within their departments.
  • Performs quality control measures to ensure data is compiled and give to supervisor to present at quality improvement committees.
  • consistently recognizes and identifies problems and recommends solutions to supervisor.
  • Logs and records Critical Incident Report data and reports to supervisor.
  • Delegates needed data collection and evaluation to staff in any department in collaboration with supervisors.
  • Collects data from all Walker Center clients throughout treatment (i.e. Point in Time Survey, Exit Survey, Admission Survey, etc.
  • Ensures the confidentiality of all client records and personal data in the use of files and the appropriateness of phone inquiries or conversations.
  • Makes an introduction during Family Program to both clients and their families to familiarize both stakeholders and those served with person calling and intent of calls.
  • Submits on-going reports starting with quarterly and eventually a final year end summary.
  • Facilitates contact with client within 5 business days of an unplanned discharge.
  • Makes post discharge contact with clients to assess adherence to discharge appointments and provides resources to clients' post discharge when barriers are identified regarding continued care needs.
  • Participates in the destruction of contraband safe items at Residential and The Walking Sober House.
  • Participates in the safety committee to plan and execute safety drills as outlined by CARF standards.
  • Assists in room searches, drug screen collection and provided additional support as needed.
