Schedule: Monday - Friday, 8am-5pm (Full-time)

Compensation: Starting compensation range of $30.00 - $45.00 hourly. Exact compensation will be determined by experience, education, licensure/certifications, and location, in accordance with applicable laws.

Position Overview:

The Patient Care Coordinator (Clinical) provides administrative and logistical support to ensure smooth and efficient patient care. This role is responsible for managing patient records, coordinating care plans, scheduling visits, and communicating effectively with patients, families, and the interdisciplinary team.

In addition to office coordination responsibilities, this position may utilize hospice clinical skills within the LVN scope of practice to support patient care operations, assist with clinical coordination, reinforce education provided by the clinical team, and respond to patient and family needs as directed.

Responsibilities:

  • Oversee the day-to-day coordination of patient care, including scheduling, communication, and ensuring adherence to the established plan of care.
  • Utilize LVN clinical knowledge and hospice experience to support care coordination, triage patient or family concerns, and communicate changes in condition to the appropriate clinical staff.
  • Assist with clinical follow-up, symptom management support, medication reconciliation, and patient/family education within scope of practice and under the direction of the RN and interdisciplinary team.
  • Provides emotional support and guidance to patients and families, helping them navigate the end-of-life journey.
  • Maintain accurate and up-to-date patient records, including medical history, care plans, physician orders, and communication logs.
  • Coordinate and schedule visits for various healthcare providers (nurses, therapists, social workers, chaplains, etc.), ensuring timely care delivery.
  • Serve as a point of contact for patients, families, referral sources, pharmacies, DME vendors, and healthcare providers, facilitating communication and resolving concerns.
  • Assist in monitoring and maintaining high standards of care, identifying areas for improvement, and implementing corrective actions when appropriate.
  • Ensure adherence to hospice regulations, standards, infection control practices, and best practices.
  • Support patient admissions, referral intake, and coordination of services, including gathering clinical information and communicating with referral sources.
  • Assist with obtaining physician orders, coordinating documentation, and ensuring timely processing of clinical paperwork.
  • Participate in interdisciplinary team meetings, case conferences, and other collaborative activities as needed.
  • Perform general administrative tasks, such as answering phones, managing correspondence, scanning/uploading records, and ordering supplies.
  • May assist with field support or patient-facing clinical tasks as needed and within LVN scope of practice.
  • Performs other job-related duties as assigned.
Requirements:
  • High school diploma or equivalent required
  • Active, unrestricted Licensed Vocational Nurse license in the applicable state
  • Minimum of 1-year hospice, home health, or related experience required; business office or administrative experience a plus
  • Proficient in Microsoft Office Suite and EMR software.
  • Excellent verbal and written communication skills.
  • Able to answer phones, maintain patient referral records, and schedule appointments.
  • Able to work closely with a variety of people while effectively using problem solving skills.
  • Strong organizational skills and able to manage multiple responsibilities simultaneously.
  • Bilingual - Spanish speaking preferred.

Work Environment & Physical Requirements

  • Regular use of computers, tablets, and mobile devices for documentation, communication, and virtual meetings; ability to sit for extended periods while completing electronic charting and administrative tasks.
  • When needed, ability to perform job duties across a variety of settings, including patient homes, assisted living communities, skilled nursing facilities, and inpatient environments.
  • Regular standing, walking, and driving between patient visits or work locations.
  • Frequent bending, stooping, kneeling, and reaching while providing care or completing assessments.
  • Ability to lift or assist with moving patients and equipment up to 50 pounds, with or without reasonable accommodation; additional support or equipment may be used for heavier transfers.
  • Sufficient hand dexterity to complete clinical tasks such as administering medications, documenting in electronic systems, and using medical equipment.
  • Visual and hearing ability to safely assess patients and communicate clearly with patients, families, and team members.
  • Ability to respond appropriately in urgent or emergency situations, including performing CPR if required.
  • Comfort working in a range of home environments, including exposure to pets, smoke, and varying indoor/outdoor conditions.
  • Reliable transportation and ability to travel within the service area.
  • Ability to manage the physical and emotional aspects of working in end-of-life care.
