Care Manager / Discharge Planner in Ashtabula, Ohio at Ashtabula County Medical Center

Summary:

Under the direction of the Director of Care Management this position develops a discharge plan for patients admitted to the inpatient and the observation setting. Coordinates the care and services of patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; assumes a leadership role within the interdisciplinary team to achieve optimal clinical and resource outcomes; facilitates and coordinates the discharge planning process.

Licensed as a practical nurse in the State of Ohio or BA in Social Work with LSW

Must be able to work independently and as part of a multi-disciplinary team.

Two years of previous health care experience preferred.
