About This Position

Job Overview:

The Equity Project Manager will work alongside Equity Department leaders to lead workforce engagement and health equity projects aimed to empowering our workforce and leaders to adopt an equity lens and be drivers of efforts to reduce health disparities. They will define requirements with leaders and usher projects through the entire project lifecycle that will promote equity within the organization. They will coordinate communications among community stakeholders and workforce on all equity efforts. Project manager will serve as liaison across teams, including operations, population health, IT, analytics, and quality. The project manager will also assist current health equity team in outreach efforts and navigation as needed.

Qualifications:

Required:

  • Bachelor’s degree

Preferred:

  • Master’s Degree
  • Experience in Equity efforts
  • 5 years of progressive experience including project management

Essential Functions:

  • Provides assistance planning, implementing, monitoring, and evaluating events and activities designed to educate and promote cultural awareness and understanding of diversity, equity, and inclusion to achieve fair and equitable outcomes
  • Provides assistance with planning, implementing, monitoring, and evaluating assigned departmental programs to ensure that decisions are evidence based and guided by data; monitors effectiveness of implemented programs and makes recommendations for improvements; develops, coordinates and implements special pilot programs and new programs upon approval
  • Through guidance of departmental leaders, will develop equity reports and newsletter to enhance awareness of all equity related programs, efforts, and initiatives to our workforce. Provides assistance with coordinating communication with community stakeholders of organizations equity initiatives
  • Cross-trained with healthcare navigators to provide patient navigation assistance and community outreach efforts as needed.
  • Provide support to Assistant Vice President and site directors in the clinical and operational integration of Health Equity, Community Impact, Diversity, Equity, and Inclusion, and all strategic priorities including the rollout of appreciative inquiry assessments and analysis for key efforts. Support day-to-day operations to complete the integration of Equity initiatives. Align communication strategies with protocol for internal and external communication related to Diversity and Inclusion. Support the execution of Equity goals and obligations such as site-specific meetings, presentations and planning of major initiatives
  • Identify areas of best practice and ensure effective delivery to appropriate stakeholders’ system-wide. Provide research support for all matters related to Equity efforts from formation of different councils, organizational committees, charter formation and appropriate communication channels. Utilize Race, Ethnicity and Preferred Language (REaL) data to identify areas of needs and report back to Program Directors and Assistant Vice President. Design and deliver health promotion and risk reduction services for diverse populations
  • Support the development and implementation of clinical system wide efforts to reduce health disparities. Working with the Medical Staff, Nursing Education Departments and Nursing Informatics throughout the system, implement and deliver a curriculum that teach nurses, medical students and new physicians how to effectively and respectfully deliver health care to the increasingly diverse populations absent of implicit biases. Provide sensitivity and education training to nurses and clinical staff for favorable and equitable outcomes
  • Reduce health disparities in alignment with population health initiatives to provide community education and other targeted chronic illnesses for populations served throughout service areas
