About This Position
Position summary: The Limb Salvage Program Coordinator is a temporary grant -funded position limited to the grant period of November 2025 through November 2028. The Limb Salvage Program Coordinator is responsible for coordinating and implementing program services, ensuring patient-centered care and compliance with regulatory and national guidelines. This role serves as a liaison among physicians, nurses, allied health professionals, patients, and families, facilitating care across the continuum and supporting evidence-based practice. Key responsibilities include identifying diabetic and peripheral vascular disease patients at risk for lower extremity complications, enrolling eligible patients into the Limb Preservation Navigation Study, and employing comprehensive disease management strategies such as care coordination, patient and family education, and resource utilization. The Limb Salvage Program Coordinator also conducts follow-up interviews, provides resources and access as needed, ensures compliance with study data requirements, and represents PVHMC in collaborative meetings and reporting to IRB, medical staff, and Nursing Governance. In addition, the position contributes to quality improvement initiatives, maintains teaching responsibilities, and fosters a positive environment through effective communication and collaboration. May perform other duties as assigned.
Required Qualifications: Valid RN license and registration with the California Board of Registered Nurses. Bachelor's Degree. BLS. Five years of recent clinical experience as RN in acute care clinical setting. Functions with the ability to develop effective action plans and implement to successful conclusion.
Preferred Qualifications: Master's Degree in Nursing. ACLS Certification; Specialty Certification a plus, i.e. Critical Care RN (CCRN), Operation Room RN(CNOR) or RN Diabetes Certified Educator (CDE); national certification as a Nurse Practitioner. Proficient computer skills in the use of Microsoft applications and PowerPoint.
Salary range: $61.89 - $87.08 hourly
