About the Role:

The Wellness Coordinator is responsible for promoting the overall health, well-being, and quality of life of individuals served under Indiana's Medicaid Home and Community-Based Services (HCBS) Waiver programs. This role focuses on developing and monitoring wellness plans, coordinating services with interdisciplinary teams, and supporting clients in achieving personal health and behavioral goals.

Minimum Qualifications:

  • Licensed Practical Nurse (LPN) license in the state of practice.
  • Experience in medical case management or a related field.

Preferred Qualifications:

  • Experience working with individuals with disabilities, mental health needs, or elderly populations

  • Experience in community health or public health initiatives.

  • Familiarity with Indiana Medicaid Waiver programs (such as CIH & FSW) preferred

  • Strong organizational and interpersonal skills

  • Ability to document services and maintain confidentiality in accordance with HIPAA

  • CPR/First Aid certification (or ability to obtain within 60 days)

  • Valid driver’s license and reliable transportation

  • Registered Nurse (RN) license in the state of practice.

Responsibilities:

  • Assess participant wellness needs as part of individualized person-centered plans

  • Develop and implement wellness strategies (e.g., physical activity, nutrition, stress management, behavioral support)

  • Collaborate with case managers, guardians, DSPs, and other service providers

  • Monitor progress toward wellness goals and adjust strategies as needed

  • Ensure services align with Medicaid waiver requirements and documentation standards

  • Promote the use of community resources and inclusion in wellness activities

  • Track and report data for audits, care team meetings, or Medicaid compliance

  • Participate in internal meetings, quality reviews, and care coordination activities

Skills:

  • Knowledge of trauma-informed care or behavior support practices

  • Experience with Therap or similar EHR systems

  • Medical case management

The required skill of medical case management is utilized daily to assess patient needs and coordinate care effectively. This involves analyzing patient data, collaborating with interdisciplinary teams, and ensuring that all aspects of a patient's health are addressed. Preferred skills, such as wellness coaching, enhance the ability to motivate and educate patients about their health choices. Strong communication skills are essential for building rapport with patients and facilitating discussions about their wellness goals. Additionally, organizational skills are crucial for managing multiple patient cases and tracking progress within wellness programs.
