Accountable for human resource, operational, financial, and patient care management. Must be knowledgeable about heart failure care throughout the continuum of care.

Communication: Daily with internal and external customers including patients, families, physicians and other service lines to coordinate quality patient care. Frequently with other managers, service lines, outlying delivery sites and external organizations to resolve problems and plan long range activities.
  • Manages and directs activities of the Advanced Heart Failure program. Responsible to VAD program and accreditation.
  • Supervises Heart Failure Clinic department personnel, including hiring, training, scheduling, and performance evaluation.
  • Partner closely with cardiology, cardiothoracic surgery, department of medicine, and other key physicians and/or stakeholders to to facilitate heart failure initiatives, promote the advanced heart failure program, and improve the care of the patient.
  • Coordinates the development and operation of the Advanced Heart Failure Program to improve the care of the patient with heart failure across the continuum of care for the system.
  • Works collaboratively with Advanced Heart Failure providers to establish standardized interdisciplinary care protocols to minimize variation in care and processes.
  • Autonomously meets with continuum leaders on ad hoc basis to revise existing processes to improve workflows for our patients
  • Establishes and oversees the pathway for patients going from hospital or Skilled Nursing Facility to Home Health
  • Partner with appropriate resources to develop a palliative & hospice program for advanced heart failure patients.
  • Facilitates communication and transitioning of care from clinic to hospital, through hospital discharge and throughout the continuum to eliminate communication gaps
  • Evaluates program success through patient interviews, retrospective chart reviews, and data analysis
  • Collaborate with CVI Quality to ensure regulatory and accreditation compliance, quality, and patient safety.
  • Plans strategies to reduce length of stay, turnaround time, resource consumption, improve patient outcomes, and other organizational initiatives.
  • Collaborates with medical staff and operational leadership to facilitate evidence-based quality and patient safety initiatives; engages associates at all levels in continuous pursuit of improvement opportunities
  • Manages, coordinates, and providers oversight of key functions and processes for the systematic, coordinated, and continuous improvement of patient care delivery
  • Coordinates and facilitates the use of statistical process tools and process improvement methodologies for continuous improvement in patient care and outcomes

Serves a guide for the growth and development of other staff members towards specified patient centered organization goals and objectives

What We Require:

Education: Graduate of approved School of Professional Nursing. BSN required. MSN preferred.
Experience: Management experience preferred.
Licensure, certification, and/or registration: Must maintain lecensure as an RN in the State of Virginia. Certification within specialty area required; CHFN certification required within 1 year of hire.
Other Minimum Qualifications: Effective interpersonal, communication, and team working skills required. Must possess analytical business skills and ability to resolve complaints/problems in a customer focused manner. Must successfully complete orientation for position.
