Position Overview We are seeking an experienced Home Health Occupational Therapist to provide skilled, patient-centered therapy services in patients’ homes. This role offers clinical autonomy while collaborating with an interdisciplinary care team to deliver high-quality outcomes.
Responsibilities Perform comprehensive OT evaluations and functional assessments Develop and implement individualized treatment plans Provide skilled OT interventions to improve ADLs, mobility, and safety Educate patients and caregivers on exercises, safety, and injury prevention Monitor progress and modify treatment plans as needed Document visits accurately and timely in EMR Collaborate with physicians, nurses, and care teams Maintain compliance with HIPAA, Medicare, and agency policies Supervise COTAs as assigned
Qualifications Graduate of an accredited Occupational Therapy program Active DC Occupational Therapist license Minimum 3 years OT experience (home health or SNF preferred) Strong clinical judgment and communication skills EMR experience preferred (WellSky/Kinnser or Homecare Homebase) Reliable transportation and valid driver’s license
