Job Title: Resident Services Coordinator

Reports to: Director of Health and Wellness

FLSA Status: Hourly

Position Scope: The Resident Services Coordinator (RSC) supports daily operations, service coordination, and resident experience. This role ensures residents receive safe, person-centered support while maintaining compliance with Evolve Senior Living standards and state requirements. The RSC partners with residents, families, and internal teams to coordinate services, support engagement, and maintain a secure, structured environment for individuals living with cognitive impairment.

RESPONSIBILITIES

Culture, Communication & Team Support

  • Supports the mission, core values, and goals of Evolve Senior Living, upholding and promoting company culture and vision.
  • Promotes a positive and professional image through actions, communication, and dress.
  • Demonstrates teamwork and collaboration with all departments.
  • Utilizes EOS principles to support accountability and communication within the Care team.

Resident Services Coordination

  • Serves as a primary point of contact for residents and families regarding service needs, updates, and general support.
  • Coordinates resident service requests, including transportation, therapy visits, vendor appointments, and follow-up on concerns.
  • Tracks and documents service needs, patterns, and follow-up actions.
  • Supports residents in maintaining routines, comfort, and engagement appropriate to their cognitive level.
  • Communicate changes in resident needs to the Director of Health & Wellness.

Move-In, Transition & Family Support

  • Coordinates move-ins, including assessments, room readiness, orientation, and family education.
  • Conducts family meetings, care plan reviews, and transition discussions.
  • Provides timely communication regarding changes in condition, incidents, or care needs.
  • Supports transitions to higher levels of care or hospice when needed.

Programming & Engagement Support

  • Supports Care programming to ensure activities are meaningful, dementia-appropriate, and aligned with resident abilities.
  • Partners with Life Enrichment to ensure daily engagement is structured, consistent, and well-documented.
  • Encourages resident participation and gathers feedback from families and staff.

Care Coordination & Clinical Support

  • Collaborates with the care team, physicians, home health, hospice, and therapy providers to support resident needs.
  • Reviews documentation for accuracy and completeness, including alert charting and incident reports.
  • Ensures follow-up on physician orders, lab results, and care recommendations.
  • Supports communication between the care team and the Health & Wellness team.

Operational & Administrative Duties

  • Maintains accurate resident files, service records, and communication logs.
  • Assists with distributing notices, calendars, and updates to families.
  • Supports staffing coordination by communicating schedule needs or changes to the DON.
  • Helps coordinate housekeeping, dining, and maintenance needs for residents.
  • Participates in a rotating on-call schedule as assigned.

QUALIFICATIONS

  • Experience in senior living, memory care, assisted living social services, or customer service preferred.
  • Strong communication, organization, and time-management skills.
  • Ability to prioritize multiple tasks and meet deadlines.
  • Good judgment and follow-through.
  • Proficiency with computers, documentation systems, and general math.
  • Ability to build rapport with residents and families.
  • Must have strong command of the English language, sufficient to speak, read, and write.

PHYSICAL QUALIFICATIONS:

  • Walk/Stand/Sitting – must be able to continuously perform duties that incorporate walking, standing, or sitting.
  • Environment Condition – must be able to perform work both inside and outside.
  • Lift - constantly 0 – 10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds.
  • Carry - constantly 0 – 10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds.
  • Push - constantly 0 – 10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds.
  • Climb – must be able to climb stairs on a frequent basis, with the opportunity to utilize the elevator as an alternate resource if available.
  • Bend – must be able to bend at the waist, knees, hips and spine on a frequent basis, may require frequently manipulating weights of 25 pounds or more.
  • Twist or rotate – must be able to twist or rotate occasionally at the trunk, hips, knees, and neck, manipulating weights of 25 pounds or more.
  • Reach - may occasionally reach overhead, requiring manipulation of weight of 10 pounds or less, and below waist manipulating weight of 25 pounds or more.
  • Sensory Vision – must be able to read clearly with or without corrective lenses.
  • Hear - Must be able to hear telephone, audible alarms, bells, and signals related to resident safety with or without hearing devices.
  • Speech/Language – must have strong command of English sufficient to read and write and interpret medical and administration information.
