About This Position

SUMMARY:

The Senior Services Case Manager Assistant is responsible for assisting in developing a Plan of Care/Assessment for services and coordinating services and support for seniors experiencing Alzheimer's and related dementias and frail conditions. Providing case management services and working as a team member in maintaining such programs is essential to ensure that activities and services are provided appropriately for participants.

DUTIES AND RESPONSIBILITIES:

  • Perform outreach activities to identify participants eligible for Case Management, Respite, Chore, Supplemental Services and Adult Day services.
  • Provide program information; implement referrals and intakes for families and participants requesting services.
  • Complete preliminary evaluation for eligibility determination for Case Management, Respite, Chore, Supplemental Services and Adult Day services.
  • Develop, implement and maintain Plan of Care/Assessment for eligible participants as well as perform reassessments annually or as necessary.
  • Communicate and coordinate with chore and respite providers to arrange for meet and greets and completion of Service Agreement, and to explain co-payment rates for services.
  • 6. Assist in planning, implementing, and providing training to FRA Senior Services staff, families, caregivers, and the community.

    7.Review fee discount schedule and complete rate agreement, submit program records to Program Support Specialist to develop file and enrollment in identified program.

  • Participate in writing quarterly newsletter.
  • Responsible for on-going case management with participants.
  • Utilize crises intervention and problem-solving skills.
  • Implement discharge of participants from respective program(s) when necessary.
  • Inputs participant information and case notes into electronic medical record, m accordance with agency procedures, state and federal requirements.
  • Implement all agency policies as appropriate.
  • Attends training seminars, in-service sessions, and staff meetings.
  • Responsible for maintaining training compliance in accordance with FRA policies and procedures, state and federal regulations.
  • Provides other duties as necessary.
  • MINIMUM EDUCATION AND EXPERIENCE:

  • Bachelor’s degree in nursing, Gerontology, Social Work or related field preferred with background in long-term care and delivery of community-based services. Experience and/or specialized education may substitute for degree requirements on a year for year basis
  • One year of paid full-time work experience, with human services recipients and providers.
  • Two years' experience working with the public one-on-one and in groups, direct contact with families and/or the elderly, preferred.
  • Must meet agency insurance criteria for operating agency and personal vehicles on agency business (21 y/o and licensed 3 years)
  • KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

    Ability to communicate effectively with participants and families, assess needs and coordinate individual services. Excellent writing and verbal skills. Ability to work independently and manage time effectively. Must demonstrate advanced computer/data entry skills. Must demonstrate team facilitation skills. Must have reliable transportation and good driving record. Must possess the knowledge and ability to utilize a computer and electronic medical record (EMR) system. Ability to learn new software and hardware applications.

    LICENSES/CERTIFICATIONS REQUIRED:

    Alaska Driver's License Ce1iification in Adult CPR Certification in First Aid Geri-Care Certification

    IMMUNIZATION REQUIRED:

    Annual PPD Test

    Hepatitis B immunization (or) Waiver

    WORKING CONDITIONS:

    Medium Work: Exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

    PHYSICAL ACTIVITIES:

    Raising objects from a lower to a higher position or moving objects horizontally from position-to-position.

    Expressing or exchanging ideas by means of the spoken word. Those activities which demand detailed or important instructions are spoken to other workers accurately, loudly or quickly.

    Driving: Minimum standards required by State Law (including license). Reliable, private transportation is available on a daily basis and provides evidence of automobile insurance.

    ENVIRONMENTAL CONDITIONS:

    None: The worker is not substantially exposed to adverse environmental conditions (such as typical office or administrative work)
