About This Position

Description:

The Visiting Nurse Association is seeking a compassionate and organized Admissions Nurse to join our dedicated healthcare team. In this vital role, you will be responsible for performing initial, in-home assessments, collaborating with physicians to determine eligibility, and developing a comprehensive, personalized care plans for patients. This role acts as the critical first point of contact, ensuring a smooth transition into home care. Key Responsibilities:

  • Conduct comprehensive OASIS assessment visits to evaluate patient health status.
  • Determine clinical eligibility for home health services.
  • Develop and implement individualized, patient-centered care plans.
  • Educate patients and families on disease management and treatment.
  • Develop rapport with referral sources, including physicians, hospitals, and discharge planners.Assess patients' health conditions and develop individualized care plans in collaboration with physicians and other healthcare professionals
  • Collaborate with interdisciplinary teams to ensure seamless communication and optimal patient outcomes
  • Ensure compliance with healthcare regulations, safety standards, and agency policies

Requirements:

- Valid Registered Nurse (RN) license in the state of Iowa

- A graduate of an approved school of nursing (RN/BSN/ARNP) with a current Iowa Nursing License.
- Two years of home health or hospice or work related experience preferred. General medical knowledge related to home health and hospice services with end-of-life issues is desirable. Strong OASIS documentation skills preferred.
- Reliable car, driver's license and adequate insurance as outlined in the personnel policies is required.
- Strong clinical knowledge, critical thinking skills, and evidence-based practice expertise.
- Excellent communication, interpersonal, and organizational skills.
- Ability to prioritize tasks, delegate responsibilities, and manage multiple priorities effectively.
- Proficient computer skills, including electronic health records (EHR) and Microsoft Office applications.
- Advanced Certifications in Hospice and Palliative Nurse certification by the 3rd year of employment.
- All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List.

WORK ENVIRONMENT AND EQUIPMENT:

  • Work is performed in clients' homes with the employee driving from location to location during the work period.
  • The locations are throughout Johnson County and within a 50-mile radius of the agency.
  • This area includes Iowa City, small towns and rural areas.
  • The driving requirements include in-town and highway driving in good and inclement weather.
  • All professional staff will use laptop computers to document their work in the client homes or at the clinic sites.

SKILLS AND KNOWLEDGE/COGNITIVE DEMANDS:

  • The employee will demonstrate skill and knowledge in providing professional nursing care for the treatment of acute and chronic illnesses and in the area of wellness and preventive health care.
  • Demonstrate competence in professional decision-making skills within the parameters of the Nurse Practice Act, including the ability to make appropriate assessments, problem-solve, prioritize, establish Plans of Care and evaluate their effectiveness.
  • Organizational skills are required to manage case load appointments and necessary follow-up each day.
  • Able to communicate effectively in the English language, both orally and in writing. Ability to maintain and update professional knowledge base.
  • Able to assimilate and use agency information regarding policy and procedure changes.
  • Able to use technology effectively and appropriately, including client care equipment, i-pad/Iphone, computers, voicemail and electronic mail.
  • Perform duties of the job at clients' in a variety of home settings. The employee will be expected to participate in evening; weekend and holiday visit coverage on a rotating basis.

PHYSICAL DEMANDS:

  • The job requires the physical demands of occasional lifting greater than 50 lbs., reaching, pushing, walking, sitting, standing, stooping, squatting, walking, going up and down stairs, driving, and moving intermittently during work hours.
  • Drives in own car to and from patients' home. Work in patient(s) homes as well as agency office.
  • Ability to cope with the mental and emotional stress of the position.
  • Performs skilled physical assessments, dressing changes, IVs, injections, blood draw, assisting with personal cares, exercises/ambulation, auscultation of lung and bowel sounds, etc.
  • Equipment for client care can include oxygen equipment, suctioning, medication dispensers, laboratory screening meters, and IV pumps.
  • Fine motor coordination/dexterity, the ability to grasp, vision with or without corrective aids adequate for fine motor tasks, client assessment, and driving.
