Clinical Nurse Outcomes Coordinator RN

Here's What You Need
• Master's Degree Nursing required
• 2 years' direct patient care experience Required and
• 5 years of current hospital experience Required
• Current RN licensure or compact licensure recognized by the Texas Board of Nursing upon hire
• BCLS - Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly Required
• CPI - Crisis Prevention Intervention Training Maintained Annually 90 Days Required
• Other CNL or other professional nursing certification within 12 Months of hire required
• SSLP - Six Sigma Lean Professional Upon Hire Preferred
• Interpersonal skills, Effective oral and written skills, Collaboration and negotiation skills, and Excellent organizational skills
What You Will Do
• Collaborates with physicians and/or APRN for services needed as well as all with other allied health care providers
• Partners with physicians by engaging in rounding to discuss priority of patients and patient care needs
• Participates in interprofessional rounds, leading when appropriate
• Collaborates with care transitions team to facilitate discharges, RCB measures, review of excess days and patient throughput
• Facilitates the development, implementation, monitoring, and assessment of processes that impact patient outcomes
• Uses benchmark data from internal and external sources to identify opportunities for improvement of care and patient safety
• Incorporates the quality caring model in daily nursing decisions
• Participates in shared decision making by participating in councils as requested by nursing leadership
