This role plays a key part in engaging patients and supporting access to Advanced Care Planning services through high-quality outbound and inbound communication. You will connect with patients primarily by phone, while also using text and email channels to educate, guide, and schedule appointments that support proactive healthcare planning. The position requires empathy, strong communication skills, and the ability to build trust with an older patient population. You will work in a performance-driven yet patient-centered environment where quality interactions and compliance are essential. Success in this role comes from balancing efficiency with compassion, ensuring patients clearly understand the value of care planning. This is a remote-friendly opportunity within a mission-driven healthcare environment focused on improving outcomes and access. You will contribute directly to helping patients make informed decisions about their long-term care journey.
You will engage patients through multi-channel outreach to educate them on Advanced Care Planning and support appointment scheduling while maintaining high service and compliance standards.
- Conduct outbound and inbound calls to educate patients on Advanced Care Planning and schedule appointments.
- Respond to patient inquiries via phone, text, and email with clear, compassionate, and accurate guidance.
- Overcome patient concerns and objections by effectively communicating the benefits of care planning.
- Maintain HIPAA compliance while ensuring high-quality, patient-centered communication at all times.
- Meet key performance metrics including call volume, conversion rates, and quality standards.
- Support a consistent and professional outreach experience aligned with organizational guidelines.
- Document interactions accurately and follow internal workflows and procedures.
- Collaborate with internal teams to ensure smooth patient engagement and scheduling processes.
This role requires strong communication skills, healthcare exposure, and the ability to manage high-volume patient outreach in a structured environment.
- High school diploma required.
- 2+ years of healthcare experience.
- 1+ year of experience in telephonic outreach, customer service, or sales.
- Strong communication and active listening skills with a compassionate, patient-first approach.
- Ability to engage effectively with older adult populations.
- Comfortable working in a performance-driven environment with defined metrics.
- Proficiency with computer systems and ability to quickly learn new software tools.
- Self-motivated with the ability to work independently in a remote setting.
- Reliable high-speed internet connection meeting required technical specifications.
- Flexibility to work shifts between 8 AM–8 PM EST, Monday through Saturday.
- Bilingual English/Spanish and prior remote work experience are a plus.
