Community Medical Group is seeking an energetic and relationship-driven Community Outreach Specialist to help strengthen our presence throughout the communities we serve.

In this highly visible role, you will build meaningful partnerships, coordinate community events, generate new member opportunities, and help ensure current and prospective patients stay connected to the care and resources they need.

What You’ll Do

• Build and maintain relationships with community organizations, senior centers, faith-based groups, schools, brokers, social service agencies, and other local partners

• Develop and execute outreach strategies that support organizational growth, member acquisition, and retention goals

• Plan and participate in health fairs, enrollment events, educational sessions, retention activities, and other community programs

• Coordinate event logistics, staffing, promotional materials, lead collection, and post-event follow-up

• Represent Community Medical Group professionally while promoting our services and commitment to patient-centered care

• Collaborate with Marketing, Patient Experience, Access Centers, clinic leadership, and broker relations to ensure a smooth experience for prospective and existing members

• Assign and follow up on outreach leads while monitoring activity throughout the assigned territory

• Maintain accurate records of leads, enrollments, events, and community partnerships in Salesforce

• Track outreach results and prepare monthly and quarterly performance reports

• Ensure outreach activities comply with HIPAA, payer guidelines, privacy requirements, and applicable regulations

• Member retention and reduced disenrollment

• Community event attendance and engagement

• Member satisfaction

• Event effectiveness and return on investment

• Timely and accurate Salesforce documentation

What We’re Looking For

• Bachelor’s degree in Public Health, Social Work, Communications, Marketing, or a related field, or at least four years of equivalent professional experience

• Three or more years of experience in community outreach, healthcare sales, managed care, public affairs, or the nonprofit sector

• Strong relationship-building and community engagement skills

• Proven ability to meet lead, enrollment, sales, or retention goals

• Experience coordinating events and managing multiple community initiatives

• Excellent verbal, written, and presentation skills

• Comfortable speaking with community members, partners, and organizational leadership

• Experience using Salesforce or a similar CRM system

• Proficiency with Microsoft Office and virtual meeting platforms

• Valid driver’s license and reliable transportation

• Ability to travel frequently throughout the assigned territory

• Availability to work occasional evenings or weekends based on community event schedules
