Job Title: Outreach Worker
Location:

Hours Per Week: 24
Schedule: Part Time 9am-3:30pm

SUMMARY:

The Outreach Worker will support the Program Manager with the coordination of community-based support and engagement services to individuals and families throughout Wayne County. The Outreach Worker will manage established partnerships to ensure schedules; referrals and marketing protocol are supported. The Outreach Worker ensures events are visible on all platforms, including but not limited to, websites, social media and print with distribution to local agencies, health centers, retailers and town chambers.

RESPONSIBILITIES:

  • Conduct outreach activities to identify and engage individuals in need of services.

  • Develop and maintain relationships with local organizations, agencies, and community leaders.

  • Organize and participate in community events, workshops, and educational sessions.

  • Maintain accurate records and submit required reports in compliance with grant requirements.

  • Travel throughout rural areas to attend community meetings.

  • Provide information and referrals to community resources, health programs, and social services.

Minimum QUALIFICATIONS:

  • Associate’s degree

PREFERRED QUALIFICATIONS:

  • 3 years of administrative office experience in healthcare.

  • Excellent oral and written communication and interpersonal skills.

  • Knowledge of rural community resources and challenges.

  • Strong organizational and problem-solving skills.

  • High level of proficiency with PC, software applications, and reporting tools.

PHYSICAL REQUIREMENTS: Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decisionmaking, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or oncall schedules are typically required.

PAY RANGE: $18.45 - $19.35

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
