Job Description

Schedule: On-call

Pay: $22.50-$24.37

General Responsibility: Under the supervision of the Program Director, the Mental Health Worker is responsible for providing direct care within a psychosocial rehabilitation program (residential program) for adults with psychiatric and co-occurring disorders (including substance abuse) with a mission to assist clients in moving towards housing in less restrictive settings in the community. Direct care includes responsibilities for the care and supervision of clients and client-related activities such as teaching and modeling daily living and socialization skills, as well as assisting clients with medication management, transportation, and other treatment-related interventions, including safety planning activities. This position is responsible for implementing techniques and strategies for assisting clients with achieving their goals and objectives.

Specific Responsibilities:

  • All shifts, including On-Call, Day, PM, and Overnight, are awake shifts and require full attention to the clients, facility, and assigned tasks.
  • Responsible for direct care of clients, and assists with case management for clients.
  • Participates in treatment planning and crisis intervention as needed, consulting as appropriate.
  • Teaches and monitors activities of daily living; provides hands-on instruction and guidance. This includes daily, weekly, and other household chores, including cleaning, transportation, meal preparation and cooking, assisting clients to understand and follow the program guidelines, and various other activities.
  • Teaches, guides, and assists clients in menu planning, shopping, and preparing meals.
  • Monitors and documents treatment/program progress, timely, accurately, and in accordance with agency policies and procedures, CCL regulations, and DMH standards and policies. This may include progress notes, weekly summaries, individual and/or group notes, incident reports, rounds, and medication accountability records. All documentation is expected to be completed using agency electronic tools and software, as well as those that are done outside of the computer EHR. Documentation is to be filed completely and accurately.
  • Participates in treatment team meetings, case reviews, staff meetings, supervision sessions and trainings as assigned.
  • Responsible for reporting of unusual occurrences, as per agency policy and procedure and CCL and DMH regulations.
  • Monitors clients’ self-administration of prescribed medications as per agency policy and procedure,including related timely and accurate reporting and documentation.
  • Assists clients in developing self-advocacy skills and empowerment by accompanying client toappointments and/or by assisting with phone calls.
  • Provides transportation in agency vehicle for clients to attend outings, meetings or other therapeutic ormedical activity/appointments. Drives agency vehicle for other agency business as assigned. Although notcommon, you may also be directed to transport clients in your personal vehicle in emergency situations (suchas in the emergency evacuation of site).
  • Leads and monitors activity program that encourages and promotes wellness and socialization.Activities take place within the program and in the general community.
  • Monitors and reports facility conditions to ensure the health and safety of clients and compliance withtitle 9, 22, 17.
  • Responds to individual client needs as deemed beneficial by the treatment team.
  • Always treats clients with dignity, respect, courtesy and communicates in an adult-to-adult manner.
  • Other related duties as needed
  • Must have graduated from high school or possess a GED and have a minimum of one (1) year of full- time experience, or its part-time equivalent, working in a program serving people with mental disabilities; experience of less than (1) year may be considered. If experience is less than 1 year of direct MH, an individual plan of supervision and training will be developed by the supervisor.
  • Experience must be in the direct provision of services to clients.
  • Able to work interdependently, constructively and effectively with others in a community-basedtreatment setting.
  • Understanding of, and/or sensitivity to the issues involved in providing service to a multi-ethnicpopulation.
  • Ability to apply critical thinking skills, and problem-solving skills.
  • Able to multitask effectively in a fast-paced environment.
  • Willing to work evenings and weekends as needed;
  • Good oral and written communication skills.
  • Some knowledge of laws, regulations, and policies governing licensed, certified and/or accredited mental health or other social service agencies or ability to learn and apply them as appropriate.
  • Must have and maintain valid driver’s license, applicable auto insurance, and a good driving record which meets the agency’s insurance carrier’s standards.
  • Must be at least 21 years of age.
  • Able to obtain background clearance, in accordance with Community Care Licensing requirement.
  • Must have health screening with negative TB test results.
  • Must maintain First Aid Certification.
  • Must be willing and able to participate in a minimum of 20 hours of in-service trainings per year and collaborate with your supervisor in one on one or groups meetings as assigned.
