Job Description

Diabetes Educator

  • JOB RELATIONSHIPS
    • Responsible to the Director of Employee Health and Allwell. Responsibilities will be split between clinical patient care/
      education, staff education, and the employee wellness program. Works in conjunction with the staff, patients ,patient families,
      and physicians to improve outcomes through education with diabetes management and all other disease specific education.
  • DEFINITION OF POSITION
    • The Diabetes Educator oversees the Diabetes Education Program at Carteret Healthcare. The Diabetes Care and Education
      Specialist (DCES) delivers high-quality, evidence-based diabetes self-management education and support (DSMES). This dualfocused role works directly with inpatients and outpatients, designs and leads clinical in-service education for hospital nursing
      and clinical staff, and collaborates with the Employee Wellness Program to promote preventive health, metabolic management,
      and lifestyle wellness across the organization.
  • QUALIFICATIONS
    • Professional
      • Registered Dietitian or Registered Nurse with current North Carolina license
      • CDCES(Certified Diabetes Care and Education Specialist) certification or BC-ADM (Board Certified- Advance
        Diabetes Management) required or obtain within 6 month of hire.
      • Previous experience in program management
      • Experience in the care of persons with chronic disease and adult education or staff training.
      • BLS Certified
      • Assumes responsibility for continued professional growth and development as required by licensure and/or professional credentialing organization
        • Attends required hospital competencies annually
        • Maintain required continuing education units as required by professional licensure and/or professional credentialing organization.
        • Maintains required continuing education in diabetes management, behavioral interventions, teaching and learning skills and counseling skills annually per ADA requirements for ADA recognition.
        • Attends all Diabetes Advisory Board meetings.
