About the Role

Lasata Care Center is seeking an experienced RN Lead Nurse Coordinator (MDS) to oversee the facility’s Resident Assessment Instrument (RAI) process and ensure accurate, compliant, and timely MDS assessments. This role is key to driving quality outcomes, regulatory compliance, and strong clinical documentation across the nursing department.

What You’ll Do
  • Coordinate all MDS, CAAs, PDPM, and care planning processes
  • Ensure timely and accurate MDS submissions (including 2x weekly transmission)
  • Oversee ARD scheduling, assessments, and facility roster management
  • Audit clinical documentation and ensure compliance with federal/state regulations
  • Lead quality measure improvement initiatives
  • Provide oversight, training, and support for ICD-10 coding and clinical documentation staff
  • Respond to audits and ensure accuracy of resident records
  • Supervise and support MDS Nurse and/or Health Information Technician (HIT)
  • Participate in interdisciplinary clinical and Medicare meetings
What We’re Looking For
  • Active Wisconsin RN license required
  • Graduate of an accredited nursing program
  • Strong experience in long-term care or skilled nursing facility setting
  • Prior supervisory or leadership experience preferred
  • Knowledge of MDS 3.0, PDPM, and regulatory compliance requirements
  • Strong documentation, organizational, and computer skills
  • AANAC certification required within 1 year of hire
  • Work Environment

    This role includes a mix of office-based work and clinical oversight, with occasional floor nursing duties. Evening/weekend work and on-call rotation may be required.
