Float B Registered Nurse - NOC Shift

The Float B Registered Nurse provides flexible clinical support across Unity Hospice Residence, Triage, and Community settings based on patient and staffing needs. This role works closely with the primary care team and interdisciplinary team to provide safe, compassionate, and responsive care to hospice patients and families.

Schedule: This is a full-time, 0.8 FTE position working 32 hours per week, consisting of four 8-hour overnight shifts from 11:00 PM–7:00 AM. The nurse will be trained to provide support in Triage, Hospice Residence, and Community settings, with the Hospice Residence serving as the home base for all shifts. The schedule includes a minimum of every-other-weekend coverage, with the potential to transition to approximately every fourth weekend once the staffing structure is established. The role also participates in an on-call rotation of approximately 10–15 shifts per year and includes 1–2 holidays per year.

Provide nursing assessments, symptom management, medication administration, and appropriate interventions based on patient needs. Maintain and update individualized plans of care and communicate changes with physicians, caregivers, families, and the interdisciplinary team.

Respond to urgent patient and family needs, provide telephone triage, initiate crisis care when appropriate, and complete new patient admissions as needed. Document assessments, orders, communications, and care provided in the electronic medical record.

Administer and titrate medications according to provider orders, manage patient and resident medications, coordinate prescriptions and refills, and provide education related to disease progression, medications, and symptom management.

Delegate appropriately to LPNs and CNAs, coordinate with team members and volunteers, and adjust priorities as patient and organizational needs change.

Successful candidates should have strong communication, organization, clinical judgment, and problem-solving skills, along with the ability to adapt in a fast-paced and changing environment. Computer proficiency and the ability to maintain confidentiality are also required.

Minimum Qualifications & Certification/Licensure:

1. Associate degree in Nursing required, bachelor’s degree preferred.
2. RN license in the state of practice required (RN must practice under the law and regulations governing nursing in the state of practice).
3. Valid WI Drivers’ License, reliable transportation, and proof of current automobile insurance coverage.
