Job Description

The Corrective Action Plan (CAP) Specialist at Zuriel LLC plays a crucial role in ensuring that
service providers comply with state and federal regulations by developing, reviewing, and
monitoring corrective action plans. This position requires a detail-oriented professional with
strong analytical skills and a thorough understanding of behavioral health and developmental
services. The CAP Specialist works closely with providers, internal teams, and external
stakeholders to address compliance issues and implement effective corrective measures.
Key CAP Responsibilities:
 Corrective Action Plan Development: Collaborate with service providers to develop
comprehensive and effective corrective action plans in response to identified deficiencies
or non-compliance issues.
 Compliance Monitoring: Track and monitor the implementation of corrective action
plans to ensure timely resolution of compliance issues. Provide ongoing support and
guidance to providers during the implementation phase.
 Audit and Review: Conduct audits and reviews of service providers' operations to
identify areas of non-compliance and potential risks. Ensure that all corrective actions are
aligned with DBHDS policies and regulatory requirements.

 Training and Education: Provide training and technical assistance to service providers
on developing and implementing corrective action plans. Educate providers on regulatory
requirements and best practices.
 Documentation and Reporting: Maintain accurate and up-to-date records of all
corrective action plans, including tracking progress and outcomes. Prepare reports on
compliance issues and CAP effectiveness for internal review and submission to
regulatory bodies.
 Collaboration: Work closely with other Zuriel/DBHDS departments, including quality
assurance, compliance, and legal teams, to ensure a coordinated approach to compliance
and corrective action planning.
 Stakeholder Communication: Serve as a point of contact for service providers,
regulatory agencies, and other stakeholders regarding corrective action plans and
compliance issues.
 Continuous Improvement: Identify trends and areas for improvement in corrective
action processes. Recommend and implement changes to enhance the effectiveness of
CAPs and overall compliance efforts.
The Quality Assurance Specialist is responsible for ensuring the highest standards of care and
compliance within our developmentally disabled group home. This role involves conducting
audits, monitoring services, and implementing quality improvement initiatives to ensure that all
services provided meet regulatory requirements and best practices. The specialist will work
closely with management, staff, and external agencies to promote a safe and supportive
environment for all residents.
Key QA Responsibilities:
 Compliance Monitoring: Regularly audit group home operations to ensure compliance
with federal, state, and local regulations, including Medicaid guidelines and DBHDS
standards.
 Documentation Review: Oversee and review resident records, incident reports, and care
plans to ensure accuracy, completeness, and adherence to policies.
 Training & Development: Identify training needs and assist in the development and
delivery of training programs for staff to ensure understanding of quality standards and
regulatory requirements.
 Quality Improvement: Develop and implement quality improvement plans, including
corrective actions for identified deficiencies and areas for enhancement.
 Resident Care Oversight: Ensure that residents receive care that is person-centered,
respectful, and meets their individual needs and preferences.
 Incident Management: Investigate incidents and complaints, conduct root cause
analysis, and ensure appropriate corrective actions are implemented and documented.
 Policy Development: Assist in the development, review, and updating of group home
policies and procedures to ensure they reflect current regulations and best practices.
 Data Analysis & Reporting: Compile and analyze quality data, prepare reports, and
present findings to management to guide decision-making and policy updates.

 Stakeholder Communication: Liaise with external auditors, inspectors, and regulatory
agencies during reviews and audits, ensuring that all necessary documentation and
responses are provided.
Qualifications:
 Education: Bachelor’s degree in Healthcare Administration preferred, Social Work,
Nursing, or a related field. A Master’s degree is preferred. Individuals with job-related
experience will be considered.
 Experience: Minimum of 3-5 years of experience in quality assurance, compliance, or a
related role in a healthcare or group home setting, preferably with developmentally
disabled individuals.
 Knowledge: Strong understanding of state and federal regulations, including DBHDS
and Medicaid standards, as they relate to group homes for individuals with
developmental disabilities.
 Skills:
o Excellent analytical and problem-solving skills
o Strong attention to detail and organizational abilities
o Effective communication and interpersonal skills
o Ability to work independently and as part of a team
o Proficiency in Microsoft Office Suite and quality management software
 Certifications: Certified Professional in Healthcare Quality (CPHQ) or similar
certification is preferred.
Working Conditions:
 This position requires regular site visits to group homes and may involve occasional
evening or weekend work to accommodate audits, training sessions, or urgent issues.

 The role may involve exposure to stressful situations and the need for quick decision-
making to resolve issues in a fast-paced environment.

Physical Requirements:
 Ability to travel between group home locations as required.
 Must be able to lift and carry up to 25 pounds.
 Prolonged periods of sitting at a desk and working on a computer.
Work Environment:
 Location: Administrative office and multiple site locations.
 Travel: Occasional travel may be required to conduct on-site audits and provide training
to service providers.
