Job Title: Licensed Practical Nurse I – Integrated Care

Reports to: RN Program Manager – Integrated Care

Accountable to: Director of Nursing

Location: Main Building, 151 N. Main St., Decatur, IL 62526

Schedule: Monday through Friday, 8:00 am–5:00 pm

FLSA Status: Non-Exempt

Position Type: Full-Time

Work Setting: In-Person

Position Overview:

In keeping with the Mission, Vision, and Corporate Values of Heritage Behavioral Health Center, the Licensed Practical Nurse I – Integrated Care provides nursing services within Heritage’s Integrated Health Clinic, which delivers primary and psychiatric healthcare services to individuals with serious mental illness and chronic health conditions. This position plays a vital role in ensuring patients receive safe, timely, and compassionate care.

The LPN I works under the direction of the RN Program Manager and alongside the LPN II team members, who provide day-to-day workflow coordination, guidance, and mentoring. This position collaborates closely with providers, nurses, medical assistants, pharmacy partners, and behavioral health staff as part of an integrated care team.

This position involves regular, direct contact with individuals living with mental illness, substance use disorders, and chronic health conditions. A compassionate, professional, patient-centered approach is essential.

Core Responsibilities:

Clinical Care & Patient Support

  • Accurately obtain and document vital signs, including blood pressure and other key indicators
  • Administer prescribed medications (weekly or as ordered) and report any adverse reactions to medical staff
  • Monitor health status and ensure timely, accurate charting in medical records
  • Communicate changes in patient condition clearly to providers
  • Provide compassionate, respectful, and professional support to patients and families
  • Maintain readiness of clinical supplies, medications, and equipment

Care Coordination

  • Coordinate with pharmacy partners to ensure timely, accurate medication refills and adjustments
  • Facilitate referrals and link patients to appropriate healthcare services
  • Assist with triage, assessment, and coordination of care for urgent health concerns by collaborating with healthcare professionals
  • Perform other duties as assigned consistent with this role

The Licensed Practical Nurse I will be successful in this role when:

  • Patients receive timely, coordinated, and compassionate nursing care within their scope of practice
  • Vital signs, medication administration, and health monitoring are documented accurately and on time
  • Changes in patient condition are communicated to providers promptly and clearly
  • Medication refills, prior authorizations, and pharmacy coordination are handled accurately and without delay
  • Clinical supplies, medications, and equipment are consistently ready for each clinic day
  • Providers and team members view this person as reliable, responsive, and communicative

Education and Experience:

Required:

  • Active, unencumbered State of Illinois LPN license
  • Current CPR certification, or ability to obtain upon hire, with certification maintained throughout employment
  • Valid driver’s license, reliable transportation, and proof of current automobile insurance

Preferred:

  • Experience in behavioral health, integrated care, primary care, or community health settings
  • Experience serving individuals with serious mental illness or co-occurring conditions

Note: Equivalent combination of education and experience will be considered.

Physical Requirements and Working Conditions:

  • Ability to stand, walk, and move throughout the clinic for extended periods, with frequent patient interaction
  • Ability to safely assist and support patients, including during urgent medical situations
  • Potential exposure to bloodborne pathogens and other clinical environments; adherence to universal precautions and proper use of personal protective equipment required
  • Regular contact with individuals experiencing serious mental illness, substance use challenges, and chronic health conditions
  • Ability to respond calmly and effectively in crisis, code medical, or emergency situations
  • Valid driver’s license, reliable transportation, and proof of current automobile insurance required
  • Ability to obtain and maintain CPR certification
  • Must adhere to all Heritage policies, procedures, and safety standards
