Job Description
Position Summary:
The Case Management Assistant (CMA) serves as the central point of contact for Case Management (CM) Staff, vendors and patient families. The CMA receives and triages phone calls on multi-line phone system and receives walk-ins. Assist CM by retrieving signatures on required paperwork i.e. Medicare Form, Observation Form and Medicare important Message Form. Assists patient family members by communicating referral information, inpatient follow-up, and case manager information. Responsible for various CMA related projects that may change over time but may include computer data input, payroll tracking, supply inventory and ordering, meeting and resource scheduling; provide information for discharge planning, maintaining meeting minutes; and monthly reports.
Qualifications
Education:
- High school diploma required.
- Associates degree in Social Work, Family Service or closely related field preferred.
Experience:
- Two years’ prior experience working in an office environment required.
- Prior experience working in Healthcare environment preferred.
- Previous social work experience preferred.
