Job Description

The Medical Team Case Manager/MOUD Coordinator at Key Recovery and Life Skills Center plays a crucial role in coordinating patient care and ensuring that each individual receives comprehensive treatment for addiction and any co-occurring disorders. This position works closely with the medical, and therapeutic teams to create, monitor, and adjust care plans for patients throughout their recovery process.

Case Management and Patient Care Coordination:

  • Act as a liaison between the patient, medical team, and other support services to ensure integrated, holistic care.
  • Monitor patient progress and implement care plans as directed by the medical team.
  • Coordinate with other agencies to ensure all outside healthcare needs are met, with particular attention to MAT/MOUD needs of patients.
  • Facilitate referrals to specialists, external medical providers, or community resources as needed for patients' continued care.

Multidisciplinary Team Collaboration:

  • Work closely with physicians, therapists, nurses, addiction counselors, and other healthcare professionals to coordinate treatment strategies.
  • Attend and actively participate in multidisciplinary team meetings, including treatment planning sessions and case reviews.
  • Ensure continuity of care between different departments, ensuring that all team members are updated on patient progress and adjustments in treatment plans.
  • Cultivate and maintain good relationships with other agencies that serve our patients, which may include meeting with those agencies.
  • Initiate and schedule any necessary appointments with our providers, as well as with outside agencies.

Treatment Plan Implementation and Monitoring:

  • Collaborate with medical staff to address complex or emerging health needs, including managing withdrawal symptoms, mental health crises, and other physical conditions.
  • Regularly assess and document patient progress in recovery, implement treatment plans as necessary based on patient feedback, team input, or clinical indicators.

Advocacy and Support:

  • Act as an advocate for patients, ensuring they have access to the resources, services, and treatments necessary to support their recovery.
  • Provide education and support to patients and their families regarding addiction, recovery processes, and available services.
  • Help patients develop life skills, coping mechanisms, and relapse prevention strategies as part of their treatment.

Documentation and Compliance:

  • Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and outcomes.
  • Ensure that all case management documentation complies with healthcare regulations, accreditation standards, and privacy laws (such as HIPAA).
  • Review and complete necessary paperwork for insurance, authorization, and other administrative functions related to patient care.
  • Maintain detailed notes regarding patient medical and MAT/MOUD treatment.

Pre-Admission Coordination of Medication, including MAT/MOUD:

  • Actively work with referring agencies to ensure medication needs are not interrupted during the transition to our program and are continued throughout their stay at our facility.
  • Initiate communication and collaboration between outside agencies on behalf of patients.

Discharge Planning and Aftercare:

  • Work with the team to prepare patients for discharge, ensuring that they have a clear aftercare plan in place.
  • Assist patients in transitioning from inpatient care to outpatient or residential treatment, if applicable, and ensure continuity of care.
  • Provide follow-up case management to ensure that patients are connected to aftercare resources and support networks.
  • Coordinate transfer of MOUD/MAT if necessary, at discharge.

Education:

  • Current Active Washington State Medical Assistant Certification.
  • Case management certification (e.g., CCM, CMC, or AADAC) is advantageous.

Experience:

  • Minimum of 2-3 years of experience in addiction treatment, mental health, or a related healthcare setting.
  • Experience in case management or care coordination, particularly in an inpatient setting, is preferred.
  • Familiarity with treatment modalities for addiction, mental health disorders, and detoxification processes.

Skills:

  • Strong case management and organizational skills.
  • Strong ability to track needs of multiple individuals, such as ongoing appointment scheduling, medication prescription renewal needs.
  • Knowledge of medical, psychological, and addiction treatment processes.
  • Strong understanding of Medication assisted treatment.
  • Excellent communication skills to interact effectively with patients, families, healthcare team, and other agencies as a Key Recovery representative.
  • Problem-solving and critical thinking abilities to address complex care needs.
  • Empathy, patience, and strong interpersonal skills to build rapport with patients in recovery.

Certifications:

  • CPR and First Aid certification may be required.
  • Additional certifications in addiction counseling, case management, or mental health are beneficial.

PHYSICAL DEMANDS:

The duties require sitting, standing, walking, stooping, bending, smiling, kneeling, reaching, pushing, pulling, twisting, ability to grip with hands and fingers. Ability to hear and speak conversationally, fluent in English occasionally lifting up to 25 pounds. Must be able to climb stairs without an elevator.
