Primary Job Responsibilities:
- Conduct Patient Examinations: Perform thorough examinations, obtain medical histories, and assess the health status of patients.
- Diagnostic and Therapeutic Services: Order, perform, and interpret diagnostic tests; prescribe medications and manage treatment plans.
- Consultation and Referrals: Consult with physicians on complex cases and coordinate appropriate specialty referrals.
- Patient and Family Counseling: Provide guidance on preventive health care and manage patient and family education to enhance understanding of treatment plans.
- Triage and Patient Communication: Handle patient calls, evaluate concerns, and triage effectively to ensure timely medical responses.
- Documentation and Reporting: Maintain comprehensive medical records and develop status reports to track patient progress.
- Interdisciplinary Collaboration: Actively participate in interdisciplinary team meetings to discuss patient care, treatment modifications, and progress.
- Facility Metrics Meetings: Engage in monthly meetings to discuss key facility metrics, including rehospitalization rates and acute care transfer rates, to improve facility outcomes and patient care.

- Sub-acute care facilities
- Active Physician Assistant or Nurse Practitioner License (Required)
- In person with some remote documentation flexibility
- Minimum two years of experience as a PA/NP, preferably in an acute, subacute, or rehabilitation setting.
- Salary: This is a per diem position paying $670 per shift.
