About This Position
INTRODUCTION:
Central Neighborhood Health Foundation is a Federally Qualified Healthcare Center (FQHC) committed to the Triple Aim as described by the Institute for Healthcare Improvement. Improving the US health care system requires simultaneous pursuit of three aims: 1) improving the experience of care, 2) improving the health of populations, and 3) reducing per capita costs of health care. Preconditions for this include the enrollment of an identified population, a commitment to universality for its members, and the existence of an organization (an "integrator") that accepts responsibility for all three aims for that population.
SUMMARY:
Direct Support Professional (DSP) functions as an advocate, supporter, educator, and caregiver to offer social and emotional support to adults with developmental disabilities. From assisting with daily living activities to promoting social engagement, your dedication contributes to the overall well-being and quality of life for those entrusted to your care. As a DSP, your compassion and commitment will play a pivotal role in encouraging individuals to lead fulfilling lives with a supportive community.
ESSENTIAL DUTIES AND RESPONSIBILITIES include but are not limited to the following functions:
Assisting with daily personal tasks that include basic personal hygiene and grooming, feeding, and ambulation.
Assisting with household activities such as housekeeping, laundry, bed making, and cleaning.
Aiding in meal preparations and making sure that their dietary preferences andrestrictions are observed.
Arranging transportation and accompanying clients to their medical appointment, visiting any family/friends, or any recreational activities to encourage social engagement.
Documenting and keeping track of all medical and dental appointments, including the administration of medication.
Ensuring the safety and well-being of the clients by maintaining a clean and healthy environment.
Monitoring and reporting any changes in the behavior, health, or emotional well-being of the clients.
Maintain a respectful and supportive attitude towards the individuals being served.
Participate in on-the-job training sessions to enhance skills and knowledge.
Performs other related duties as assigned.
QUALIFICATION REQUIREMENTS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION AND/OR EXPERIENCE:
Must possess DSP I and DSP II certification.
Must have prior experience with the developmentally disabled population
Must have a valid CA driver's license and the ability to transport clients.
Must be able to pass FBI/DOJ background checks.
Must be able to work with flexible shifts, including one overnight shift.

Excellent organizational and communication skills.

Ability to work independently while also being receptive to and capable of adhering to guidance and instructions when required.
Ability to handle challenging situations with professionalism and a positive attitude.
Ability to handle confidential information with discretion.
Basic computer skills for documentation purposes.
