About This Position

Description:

The Medical Social Worker conducts psychosocial assessments of patients and their families, developing and executing care plans, and interfacing with physicians, nursing staff, and community agencies to coordinate services for hospice patients.

RESPONSIBILITIES:

- Conducts psychosocial evaluation/assessment of patients/families, and develop individual treatment plan in accordance with assessment.

- Provides individual and family counseling services, outreach services, information and referrals, financial planning services, and other social services as developed in the plan of treatment.

- Provides individual and group grief and bereavement services to hospice families and other community members as requested.

- Builds sufficient medical knowledge of major condition on assigned cases to communicate and plan with the interdisciplinary team.

- Utilizes community resources. Explores and enlists informal resources where no organized resource exists.

- Maintains up to date patient records so that care plan changes are reflected as they occur.

- Attends and is an active participant in team meetings.

- Provides consultation to all other agency disciplines on work centered problems.

- Performs a minimum of 3 patient visits per day.

- Carries an average caseload of 30 patients.

- Assigned areas can change to meet the needs of the patient and the organization.

- Networks with community agencies in the interest of patients.

- Develops relationships and works collaboratively with area hospital discharge planners to assist in discharge planning for patients.

- Conducts hospice consults at hospitals, nursing homes, patient’s homes when requested.

- Must demonstrate effective communication skills with team members, patients, families, physicians and others in the health care community.

- Maintains up to date patient records so that care plan changes are reflected as they occur.

- Follows the guidelines for universal precautions when providing patient care.

- Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire, security, risk management and patient privacy rules.

- Participates with on call services as necessary.

- Performs other duties as assigned.

Job Type: Full-time

Expected hours: 40 per week

Schedule:

  • 8-hour shift
  • Monday to Friday
Requirements:

QUALIFICATIONS:

- Master’s Degree of Social Work or Bachelors of Social Work from an accredited school with current licensure in the state of KY and OH.

- Continuing education requirements will be maintained in accordance with the State of KY & OH requirements for licensure and/or certification.

- Previous work experience in human service or health care program.

- Knowledge of psychological needs of the dying and bereaved person.

- Good organizational, interpersonal and communication skills.

- Personal qualities of empathy, responsibility, dependability and intelligence.

- Must demonstrate effective communication skills with team members, clients and family members.

- Must arrange travel and be able to access client homes independently.

- Must observe universal precautions

- Must have the ability to work independent of direct on-site supervision.

- Must be able to participate in the Federal and State Healthcare Programs.

- Must be able to pass a Driving Records Check, Criminal Background Check, and a Pre-Employment Drug Screen.

Education:

  • Master’s Degree of Social Work (Preferred) or Bachelors of Social Work (Required) from an accredited school with current licensure in the state of KY and OH.

Experience:

  • Previous work experience in human service or health care program. (Required)
