Key Responsibilities Conduct comprehensive psychosocial assessments for patients and families Provide crisis intervention, counseling, and emotional support Collaborate with interdisciplinary care teams on individualized care plans Advocate for patient needs, rights, and access to community resources Assist patients and families with healthcare navigation and care decisions Coordinate referrals for housing, financial assistance, and support services Maintain accurate documentation in compliance with regulatory standards Participate in care conferences and discharge planning Stay informed on community resources and healthcare regulations
Qualifications Master’s degree in Social Work (MSW) from a CSWE-accredited program Active or eligible LMSW or LCSW licensure 2–3 years experience in medical social work or healthcare settings Strong skills in psychosocial assessment, counseling, and case management Knowledge of community resources, benefits, and healthcare systems Excellent communication, organization, and time-management skills Ability to work independently in a fast-paced home health environment Commitment to ethical practice and HIPAA compliance
