Position: Healthcare Navigator/Case Management Support

Program: Youth Resiliency Center Teens and Tots Clinic
Status: Full-Time, Hourly position.

Description:
The Healthcare Navigator for the Teens and Tots Clinic is directly responsible for providing integrated, quality, youth-focused and accessible healthcare by linking clients with medical, dental, vision and mental health services, community resources, advocating to meet client needs and completing documentation. Primary duties also include navigating healthcare systems on behalf of clients to find them the care they need. This position follows the onsite Standard Operating Procedures in all day-to-day operations.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
  • Acts as a liaison between clients, Synergy Teens and Tots Clinic partners (Children's Mercy, Miles of Smiles, Vision Source Eyecare and Synergy Therapists), other Synergy programs (Synergy House, Transitional Living, Street Outreach, Youth Resiliency Center, Maternity Group Home, Children's Center, Domestic Violence Center, Domestic Violence Housing Program, out-patient clinical), external agencies, and other community resources.
  • Assists clients with meeting basic healthcare needs, documentation, communication and aftercare; assists clients with resources as requested or needed by Synergy personnel and/or client.
  • Coordinates regular Teens and Tots Clinic case reviews with clinic providers
  • Meets daily with clients to set goals and monitor progress regarding medical, dental, vision and mental health related matters.
  • Transports clients to Teen Clinic for aftercare and current clients to community medical appointments, as needed.
  • Facilitates educational health groups for clients, as needed.
  • Offers case management support for Transitional Living Program/Maternity Group Home and Synergy House programs providing quality client care by linking clients with community resources, advocating in meeting client needs and completing documentation. Assist youth in finding housing options, jobs, childcare, education, medical services and other essential services and assist with learning basic living skills. Work with clients weekly at minimum to set goals and monitor and document progress. Complete needs assessments.
  • Participates in on-call rotation and may assist other departments as requested.
  • Attends SCM training and stays up to date on all required trainings and testing.
  • Attend community meetings, tabling events, etc. as a representative of Synergy.
  • Provide informational talks in community to explain Teens and Tots Clinic and spread the word in the area about the service
  • Documents daily on clinic client activities in Synergy's electronic database – CCL.
  • Reports data to Grant's Department as necessary.
  • Assists clients with Medicaid enrollment.
  • Assists in obtaining vaccination records/prior medical records.
  • Coordinate and share data collection on a monthly basis.
  • Assists clients with insurance matters as it relates to their care and services.
  • Maintains respectful and professional communication with other staff members – both oral and written.
  • Attends campus meetings as instructed/needed and relays relevant information to campus staff.
  • Acts as a liaison, with and between Synergy programs; includes being available for team meetings (Synergy House, Street Outreach, Transitional Living, Maternity Group Home, Permanent Housing) to relay relevant information to campus staff, as necessary.
  • Performs general clerical duties, answers telephone and hotline calls, answer clinic door.
  • Attends internal and external trainings
  • Maintains client confidentiality.
  • Assists clients in crisis.
  • Performs other duties as assigned.
SUPERVISORY RESPONSIBILITIES
This job has no supervisory responsibilities.
QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Bachelor's degree in Social Work, Psychology, Counseling or related field from an accredited college or university preferred but not required. Knowledge of medical case management and/or experience preferred but not required.
LANGUAGE SKILLS
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
MATHEMATICAL SKILLS
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
CERTIFICATES, LICENSES, REGISTRATIONS
The employee in this position must have a valid Class E Drivers license. The employee in this position must obtain CPR/First Aid Certification and be a Medical Technician.
OTHER SKILLS AND ABILITIES
The employee in this position must have the ability to transport up to fifteen (15) passengers in a van. The employee in this position must have the ability to type and perform Data Entry/10 key, and use computer software, including Windows, Word for Windows, Alice, and Access.

While performing the duties of this job, the employee is regularly exposed to fumes, airborne particles, or communicable diseases. The noise level in the work environment is usually moderate. While performing the duties of this job, the employee is regularly exposed to the risk of violence and secondary trauma.

Additional Requirements:

Must be at least 21 years of age, pass a background check and Family Care Safety Registry screening, fingerprint screen, drug screen and have a valid driver's license and proof of insurance. Position requires CPR, Med Tech Certification and Chauffeur's License within 30 days from the date of hire.
