• Administer medications and treatments as ordered by the physician in accordance with Ohio nursing regulations

• Monitor residents health status, including taking and documenting vital signs

• Perform skilled nursing procedures such as wound care, catheter care, tube feedings, injections, and blood glucose monitoring

• Assess changes in resident condition and promptly report findings to the supervising RN or physician

• Contribute to the development and implementation of resident care plans under RN supervision

• Supervise and delegate tasks to STNAs/CNAs as appropriate

• Provide direct patient care and assist with activities of daily living when needed

• Ensure accurate, timely documentation in accordance with facility and state guidelines

• Communicate effectively with residents, families, interdisciplinary teams, and physicians

• Respond appropriately to emergencies and follow facility protocols

• Maintain infection control standards and ensure safe medication administration practices

• Uphold resident rights, dignity, confidentiality, and safety at all times

• Participate in required in-service training and staff meetings

• Perform other duties within LPN scope of practice as assigned

• Active LPN license in the state of Ohio

• Current BLS certification

• Physically able to lift 50 lbs and assist residents with mobility

• Strong communication skills; ability to read, write, and speak English

• Previous experience in assisted living, long-term care, or skilled nursing preferred

• Experience with memory care residents is a plus
