Starting Pay Range –
$115,000-$145,000 annually depending on experience.
Summary:
Work in close collaboration with the Trueblood Element Programs, FPATH (Forensic Projects for Assistance in transition from homelessness), FHARPS (Forensic Housing and Recovery through Peer Services), OCRP (Outpatient Competency Restoration Program), and the DSHS (Department of Social and Health Services) FNs (Forensic Navigators), as well as the internal program MA (Medical Assistant), to provide effective medical and Prescriber services to Program Participants where they are in the community and in county/city jails as appropriate, regardless of medical insurance coverage or Medicaid status. Program Participants are typically houseless, at risk of homelessness or have been recently and/or temporarily housed.
Essential Job Responsibilities:
Coordinate with the Trueblood Element Programs (above), local jails, hospitals, crisis facilities, and other community touchpoints to ensure accessible prescribing services, formulate treatment plans, and reduce gaps in medication maintenance
Provide basic medical care services for the Program Participants, including but not limited to the following: wound care and foot care, sexually transmitted infections, Human Immunodeficiency Virus (HIV), and Hepatitis C testing, Birth control, Naloxone and overdose prevention training, vaccines, and care for acute and chronic conditions.
- Provide services without stringent requirements or policies for prescribing and accessing medications to the Program Participant.
- Provide appropriate trauma-informed approaches, harm and risk-reduction information, methods, and tools when providing services to Program Participants.
Non-essential Job Functions:
- Support agency mission, goals, and objectives.
- Attend staff and planning meetings as scheduled.
- Maintain communication and cooperative working relationships both within the agency and community at large.
- Active driver’s license and valid insurance required.
- Other duties as assigned by Supervisor.
Education, Licensing and Experience:
- An active, unrestricted Physician or Advanced Registered Nurse Practitioner
- CPR certified
- One year or more of experience in behavioral health prescribing preferred
- Experience working with the aged, the uninsured, the underinsured, individuals with severe mental health disorders, and/or substance use disorders preferred.
- Ability to credential with Medicare, Medicaid and commercial insurers
