Salary Range: $65,000 – $75,000 CAD per annum

Position Summary:This is a hands-on, field-based role being built in close partnership with the Director, Clinic Transformation. You will own a defined portfolio of GTA clinics, driving the adoption of clinical programs — including AI-powered clinical decision support tools and patient recruitment for clinical trials — through the trusted relationships the CT team has already built with physicians and clinic staff.You will not be handed a finished playbook. What You Will Be DoingClinic Portfolio Ownership
Own end-to-end execution of clinical programs across a defined portfolio of GTA clinicsDrive physician onboarding, workflow integration, go-live, and sustained engagement for each programBe the trusted field contact for physicians, MOAs, and clinic managers — the person who gets things done
Physician & Program Activation
Educate and onboard physicians into AI-powered clinical decision support tools and physician participation programsSupport patient identification and referral workflows for clinical trial recruitment within your portfolio clinicsFollow up consistently to keep physicians active — adoption is an ongoing outcome, not a launch event
Cross-Functional Collaboration
Work alongside the Director, CT and collaborate with internal teams including the WELL Health Medical Network, clinical, digital health, and operations teamsHelp bridge the gap between internal program planning and real-world clinic execution — your field feedback shapes how programs improveAs the role matures, support clinical program coordination for CT expansion into other provinces

What Success Looks LikePerformance in this role is measured by outcomes, not activity. Within your first 6 months your targets will include:Physician Activation
# of physicians enrolled and actively using clinical decision support tools within your portfolioTime to activation — days from first clinic introduction to first physician live on a programSustained engagement rate — % of physicians still active 60 days post-onboarding (not just launched and dropped)
Clinical Trial Recruitment
# of patients referred and entered into clinical trial pipelines through your portfolio clinics% of portfolio clinics with at least one active participant referral workflow in placePipeline conversion rate — eligible patients identified vs. patients referred vs. patients enrolled
Portfolio Health
% of assigned clinics with at least one active program running# of clinic visits and physician touchpoints per month — field presence is a leading indicator, not the outcomeBarrier resolution rate — issues flagged vs. issues resolved within agreed timelines
Targets will be set with the Director, Clinic Transformation in your first 30 days and reviewed monthly. This role comes with clear accountability and clear recognition when you deliver.


Who You AreYou Bring
2–5 years in healthcare operations, clinic management, customer success, clinical engagement, or a related fieldExperience working in or closely with primary care clinics — you understand how these environments operateStrong relationship-building skills — physicians and clinic staff trust you quicklyAn organized, execution-focused approach with comfort being measured on outcomesValid driver’s licence and willingness to travel regularly across the GTA
Nice to Have
Experience in clinical trials, participant recruitment, or health technology implementationFamiliarity with AI-powered clinical tools, EMR environments, or digital health platformsExposure to cross-functional or multi-stakeholder project coordination in healthcare
