About This Position
Description:
Alium Healthcare is seeking a skilled and compassionate Family Nurse Practitioner (FNP) to join our team. The ideal candidate will be responsible for providing comprehensive medical care, including examinations, diagnosis, treatment, and health education to our patients. This role requires a strong commitment to patient-centered care, team collaboration and maintaining high standards of clinical practice.
Essential Job Functions:
- Patient Care: Perform medical examinations, evaluations and diagnoses. Develop and implement treatment plans, provide follow-up care, and offer consultation as needed.
- Patient History and Documentation: Obtain and maintain comprehensive patient care charts to ensure completeness and accuracy.
- Health Education: Provide health education to patients, families and community health practitioners. Conduct educational sessions and workshops as appropriate.
- Supervision and Coordination: Supervise and Coordinate the activities of patient care and support staff. Ensure effective collaboration and communication within the care team.
- Healthcare Activities: Administer specific healthcare activities as appropriate for the facility. Participate in planning and developing health programs, policies and procedures.
- Safety and Compliance: Adhere to safety, environmental and infection control methods. Maintain high standards of quality and patient safety.
- Miscellaneous Duties: Perform other job-related duties as assigned to meet the on-going needs of the organization.

Requirements:Education Requirements:
- Master's degree in nursing with 1-3 years of experience directly related to family practice treatment.
- National Certification from the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
- Valid AZ State Nurse Practitioner license
- National Provider Identification (NPI) number and individual CAQH account
- Federal DEA license in good standing
Knowledge, Skills, and Abilities Required:
- Clinical knowledge: Knowledge of patient care charts, patient histories, and health education theory and practice. Understanding of clinical operations and procedures, including diabetes management and Pulsed Activation Therapy.
- Interpersonal Skills: Strong interpersonal and communication skills with the ability to work effectively with a diverse range of patients and colleagues.
- Leadership Abilities: Ability to supervise and train staff, organize and prioritize work assignments and ensure effective team collaboration.
- Quality and Safety: Ability to maintain quality, safety and Infection control standards. Familiarity with related accreditation and certification requirements.
