About This Position
Responsible for providing mental health services to severely and chronically mentally ill or emotionally disturbed children and their families to include dual disordered mental health/chemically dependent individuals. Provides ongoing case management services to assist assigned individuals in gaining access to medical, social, educational and other needed services. Service interventions are provided in home, school and community settings, and include screening, admission to service, consumer assessments, recovery planning, team treatment staffings, continuity of services monitoring, referral and follow-up services, individual rehabilitative services, family rehabilitative skills training, group skills and provision of crisis resolution services. Performs the duties as addressed in the Texas Administrative Code, Community Standards and policies and procedures of the agency. Required to adhere to agency productivity standards.
