About the Role
We are seeking a Nurse Practitioner or Physician Assistant to join our structured Skilled Nursing Facility (SNF) Physiatry Program, working directly alongside a board-certified physiatrist.
This is not a passive rounding role. You will help run a high-impact, quality-focused PM&R program designed to:
- Accelerate functional recovery
- Reduce avoidable hospitalizations
- Optimize therapy alignment
- Improve pain control
- Support safe, timely discharge
You will participate in structured daily rounding within designated SNFs, following standardized clinical pathways and quality protocols developed by our physiatry team, while working hand-in-hand with our interdisciplinary team and support staff.

The NP/PA will provide comprehensive Physical Medicine & Rehabilitation services for post-acute and long-term care residents, including:
- Stroke recovery
- Debility and deconditioning
- Fractures and orthopedic recovery
- Joint replacements
- Spinal disorders
- Chronic musculoskeletal pain
- Neurologic conditions impacting mobility
Your work will center on restoring function, improving mobility, reducing pain, coordinating care, streamlining transitions of care, and enhancing quality of life.
What Makes This Role UniqueStructured Program ModelYou will be part of a defined physiatry-led SNF quality program. This role is built around standardized visit cadence, outcome tracking, therapy alignment, interdisciplinary team engagement, and discharge planning support.
Direct Physician Mentorship
Work closely with an experienced physiatrist who provides oversight, education, and case-based teaching.
Formal Education & Didactics
In addition to one-on-one mentorship, our advanced practice providers participate in structured educational sessions led by our physician team. These include periodic case conferences, clinical topic reviews (e.g., spasticity management, complex pain strategies, frailty syndromes, rehabilitation optimization), grand rounds, and interdisciplinary learning discussions. We believe in developing excellent clinicians and invest in continued clinical growth across our organization.
High-Impact Clinical MedicineYou will influence:

- Active Illinois NP or PA license
- National certification (FNP-C, AGNP, ACNP, or PA-C)
- 1–3+ years experience in physiatry, geriatrics, post-acute, or rehabilitation preferred
- Skilled Nursing Facility experience strongly preferred
- Strong functional and musculoskeletal assessment skills
- Ability to work independently within a structured, team-based model
Salary: $120,000
