Salary:$32.00-$36.06/ hour

Schedule: Monday-Friday

Location: Los Gatos, CA

Summary:

The Quality Improvement (QI) Coordinator plays a key role in advancing ECHMN's quality initiatives. This position supports efforts to enhance patient outcomes, comply with regulatory standards, and meet strategic goals. The QI Coordinator is responsible for coordinating quality programs, managing payer quality initiatives, leveraging health plan portals to support care gap closure, and driving performance improvement. This role partner’s with clinical and operational teams to collect and analyze data, implement improvement projects, and promote a culture of safety and continuous learning across the organization.

Responsibilities:

  • Collects, analyzes, and reports clinical and operational data related to quality measures and performance improvement efforts.
  • Coordinates and supports quality improvement initiatives in alignment with SVMD’s strategic objectives.
  • Serves as a field liaison, providing hands-on support and guidance to clinic staff.
  • Collaborates with clinical teams to implement evidence-based practices and improve patient outcomes.
  • Prepares documentation for site visits and performance reviews; assists with follow-up actions and improvement plans.
  • Educates staff on quality metrics, data reporting requirements, and improvement methodologies.
  • Promotes the use of Plan-Do-Study-Act (PDSA) cycles and other recognized quality improvement tools.
  • Monitors and manages health plan dashboards to ensure care gap closures, timely supplemental data submissions, and compliance with value-based care requirements.
  • Supports efforts to improve the overall patient experience and satisfaction.
  • Maintains compliance with all HR, safety, HIPAA, and regulatory policies.
  • Performs work in accordance with established procedures, timelines, and standards.
  • Performs additional duties as assigned.

Minimum Requirements:

  • High School Diploma or GED
  • An associate’s degree from an accredited institution is required.
  • Minimum 1–3 years of experience in quality improvement, healthcare operations, or related clinical field.
  • Strong analytical and data management skills; experience with Excel, EHR systems, and reporting software (e.g., Tableau) is a plus.
  • Excellent communication, collaboration, and organizational skills.
  • Ability to manage multiple projects, work independently, and drive results across teams.
  • Ability to accurately type a minimum of 40 WPM.
  • Strong verbal and written communications, and active listening skills.
  • Solid patient/guest relations skills with the ability to communicate in a professional, courteous and efficient manner.
  • Ability to effectively organize and prioritize tasks to complete assignments within the time allotted to maintain standard workflow.
  • Ability to work effectively as a team member who is flexible, cooperative and willing to assist others.

Preferred:

  • EPIC EMR experience
